Need interpretation of some MRI results please

Hi. My 73 year old Mom has a history of seizures(partial I believe) but has been well controlled with Lamictal until recently where she is starting that pre-seizure "horrible feelings". Anyway she just had a MRI of her brain and I can't figure out what the results are telling me. Can anybody please help? I so greatly appreciate it. The part I can't understand is as follows: The ventricles, sulci and folia are prominent, more than usual for a patient of this age.    And the one that really atumps me...:There is extensive abnormal signal in the periventricular and corticomedullary white matter, including the midbrain, pons and cerebellum...     Then after that it says" small vessel ischthemia is likely".
