Need interpretation of some MRI results please

Hi. My 73 year old Mom has a history of seizures(partial I believe) but has been well controlled with Lamictal until recently where she is starting that pre-seizure "horrible feelings". Anyway she just had a MRI of her brain and I can't figure out what the results are telling me. Can anybody please help? I so greatly appreciate it. The part I can't understand is as follows: The ventricles, sulci and folia are prominent, more than usual for a patient of this age.    And the one that really atumps me...:There is extensive abnormal signal in the periventricular and corticomedullary white matter, including the midbrain, pons and cerebellum...     Then after that it says" small vessel ischthemia is likely". Thank you for any input!
