Hi there. This sounds like amniotic fluid embolism, where amniotic fluid enters the open venous sinuses in the uteroplacental circulation.  This is a rare obstetric emergency usually occurring during abortion, after abdominal trauma, during amnioinfusion. Usually the woman in late stages of labor becomes acutely dyspneic with hypotension , experience seizures followed by cardiac arrest, massive DIC associated hemorrhage follows this and ends in death of the parturient. Death is mostly within an hour of onset. You can check related data in the books or the internet. Take care.

We have one of the highest maternal death rates in the Western world. We also have an extremely high C-section rate. Do the math.

It is pretty much impossible to say what happened here, given the brevity of the description.

Could have been a pulmonary embolism, given the sudden and unexpected demise immediately following surgery.  This can rarely happen.

Could also have been an éclamptic seizure, and she may have aspirated and died from it, or remained in a seizure state so long she was asphyxiated.

Just hard to say.  The hgb you quoted is on the low side, but ok.
