third nerve palsy

sir,
my younger brother mr. shivam is suffering from thrid nerve palsy form 2001 when he was 4 year old.
doctors at aiims(new delhi) examined him for 1 month in hospital to get the cause. they prescribed to do CT scan , MRI , MR of his brain but reports of scan resulted nothing unordinary.
these things happened three times from 2001 to 2005.
they said there is nothing wrong in neurology.

in his case , attack of this palsy starts with two or more vomiting ,then gradual deflection of right eye towards his right and then shutting down of his pupil.
when we consult to the neurologist , they prescribe the "prednisolone tablet" called wysone with dose of
30 mg - 5 days
25 mg - 5 days
20 mg - 5days
15 mg - 5 days
10 mg - 5 days
5 mg -   5 days

along with a calcium supplement and a famotidine tablet called famocid.

he is very active and fit but
he keep facing this problem for once or twice a year.

is this normal phenomenon????

Dear Suraj, welcome to the medhelp forum. I can understand your concern about your brother’s health. One of the causes for recurrent episodes of ophthalmoplegia due to occulomotor palsy, though rare is mucocoeles of the nasal accessory sinuses. This mucus retention in the sinus causing thinning of the sinus wall. A thorough nasal examination would be needed. mucocoele can exert pressure on either the optic nerve, causing visual symptoms or temporary or permanent palsies of the occulomotor nerve. Your brother also needs an evaluation for granulomatous disease, sarcoidosis, which can cause 3rd nerve palsy and responsive to prednisolone treatment. Since he presented with these symptoms at 4 years, a rare cause could be ophthalmoplegic migraine. Does he have headaches associated with the vomiting episodes? These palsies usually resolve but repeated attacks may leave some deficit. You could discuss with the doctor treating him. Wishing all the best for your brother.

An important issue is not to wear the eyepatch over the affected eye 24/7 or the optic nerve may atrophy. Wear the eyepatch no longer than 12 hours. This problem develops when an eyepatch is worn contunually for extended periods of time.

he has been treated for sinus and migraine as well but cause was not found , any idea about a good neurologist in new delhi . he was treated by rohit bhatia (hod neurology aiims).
