Painful lips

I posted on the depression forum about my nephew, and it was suggested that he may be showing symptoms of OCD.
He seems depressed but the major problem seems to be that he says his lips are burning and swollen and peeling. He says he cannot talk to strangers because they will be looking at his lips. They are not swollen and look perfectly normal to me. He complains about it but will not do anything to help himself. I should say that my daughter had an episode of OCD with her skin problems. She had just had a baby and we kind of thought it was just hormonal. She went for help and was put on Paxil for 10 months. It helped her tremendously and she has not had to take Paxil again and has been back to her old self for 4 years. She tried to talk to my nephew about it but he just will not go for help. I wish I knew how we could help him.
