Pott's Spine

Hello,
My name is Ajay. It is basically regarding TB infection in my spine from D9 to D12. I was diagnosed for TB last year in may 2009 and i got confirmed that i have spinal TB infection in my spine. Since May 2009, i started taking medicine for  TB infection, in the beginning, I took Akt 4 tablets, becousles capsule and pyridoxine tablet for 4 months which responded very quickly and back pain disappeared and doctor prescribed me AKT2 with becousles capsule and pyridoxine tablet for rest of the time. I got no problem for the next few three months however after three months I started feeling mild pain in lower ribs and stomach on right side, I spoke with doctor about it he suggested me to go for ESR test that was 20mm/hr that was normal according to him and suggested me to continue the prescribed medicine as it in addition to LIV52 syrup but I got no relief in the my lower ribs and stomach pain and since then I’ve not got relief in my pain and I stared feeling stiffness in my back as well then doctor advised me for CT scan for whole abdomen since I got pain in stomach on right side but no abnormality was detected and after getting this test done he suggested me to get repeat MRI done for dorsal spine and reports reads as follows:

(FUC of caries spine D9 to D12)
There is evidence of diffuse marrow signal intensity alterations hyperintense  on  both T1 and T2WI in body and pedicles consistent with fatty infiltration with irregularly marginated area of low signals on both T1 and T2WI in body, likely sclerosis in D11 vertebra. There is mild pre and right paravertebral low signal intensity fibrotic soft tissue/ ALL thickening is seen in mild and lower dorsal region till D12 vertebra with mildly scalloped underlying adjacent anterolateral cortical outline of D9 to D12 vertebra. These changes are consistent with healed tubercular spondylitis.
Other vertebrae and intervertebral discs are normal.
CSF shows normal signal intensity.
Cord is grossly normal.

IMPRESSION – FUC of caries spine from D9 to D12 level reveals complete resolution of the disease process.

After reading the report, doctor says that tubercular infection has been properly healed and there is problem related to this in my body at all however I have not got relief from the pain in stomach so far hence my doctor said it may be related to any nuerological problem and advised me to take MAHAGABA capsule twice a day and now I m felling a little relief in the pain and he also advised me to stop taking AKT 2 since it has been one year now taking these medicines.

Here, I need your advice what I should do.
Shall I continue taking AKT 2 for more time or not?
Problem in my stomach and ribs can be sorted out by new prescribed medicine that is MAHAGABA M75.
Would this stiffness in the lower back go away with time or not?
Shall I discuss this problem with any neurologist now since I was told this pain and stiffness may be due to nerve in the body.
Kindly advise me what I should and where I stand at this stage.
Please help me with my queries. Your response will be highly appreciable.

Hello!

You have taken anti-tubercular drugs for 1 year and 2 months, normally 6-9 months is sufficient to heal.

If your weight gain is fine and X-ray and ESR results are fine your infection is cured.

You would need proper orthopedic assessment after the damage by the bacteria on your spine and to take corrective support. Correlate clinically with an orthopedician along with the MRI
You have to understand that the bacteria have damaged your thoracic vertebra to some extent. (T9 to T12 but now cured)

You ask some excellent questions.

First off there are several different varieties of the mtb organism, some much worse then others. "TB" is an insufficient diagnosis. Different varieties mean different protocols.

Scientific American magazine has had some excellent article on TB. Do a search.

Once infected you can only hope for remission. The disease never goes away. You will habor infection until the day you die.

The damage done by the disease and surgical intervention is another issue.

That is why you must maintain a healthy immune system, get plenty of sunlight every day (a must) and follow your physician's treatment protocols without fail.
