knee pain

Hi, my name is james and I'm 27 and I was wondering if you can help me at all for my knee. For about 6 years before my injury that happened in about 2011 I was training, competing and doing demonstrations in a sport combining martial arts and gymnastics at high level. There wasn't anything specific that I did to my knee but from then till now I have had a lot of problems, it hurts to run and even walk sometimes, its also uncomfortable most nights to sleep and to drive amongst other day to day things, kicking my leg out and locking it out hurts a lot whether it is to something like kicking a football or nothing and also when I push off to jump. its feels like the pain is coming from behind the kneecap. Also over a year ago a banged the same knee pretty hard on the floor which ever since I've been unable to kneel down on that knee not even on a bed or mat it feels like a really bad bruise and shooting pain that I get, I was originally told it was a bruised bone but that was over a year ago and is still just as bad. I've had an mri scan and its showing nothing more than abit of fluid at either side. No one seems to really know what the problem is, I've gone through lots of physio, conditioning, stretching, deep tissue manipulation, rested/iced it etc it and nothing seems to work or make it feel better. I originally wanted to get back training again but to be honest right now I want it to feel better just doing day to day things. I was hoping the mri scan would show something so I could have surgery but it hasn't so not sure what to do now. I would appreciate any help If there's anything else you need to know please ask.

Its funny you should say I should avoid postures where my knee is kept bent for long because that's exactly what I don't do, this whole time I've been doing everything I can to keep it bent including sleeping with it bent due to it being uncomfortable to keep it straight. I will however from now try and do as you say. Thanks

Thank you I will bare all this in mind, I have been told I might have a lot of things but no one ever seems to know for sure but I'm set to see a specialist at the end of the month I just hope I don't get told the same thing as the the last one which was 'I don't know so just stop doing what your doing' and discharge me. Thanks again for the reply, I will look into this.

Hi James!
If MRI is normal then the pain can be due to Patellofemoral syndrome. This happens due to mal-alignment of the knee cap on bending the knee (and hence any imaging study has to be taken with the knee bent), injury to the knee (you may have sustained it in past), flat feet, over tight or over lax muscles around the knee joint. All these aspects will have to be investigated by your doctor. Other possibilities are bursitis (inflammation of fluid filled sac over joints), tendonitis (inflammation of cords joining muscles to bones) or due to weakness of patellar bone. Please discuss all these possibilities with your treating doctor.
If it is patellofemoral syndrome, then first physical therapy is started. Other treatments include knee caps and braces, taping the patella to reduce friction, anti inflammatory drugs, custom designed foot wear to correct arch problems if any and rest to the knee joint and applying ice packs. You need to avoid postures where knee is kept bent for long. Please consult a sports injury specialist for this. Take care!
