hi doctor,
my mother(44 years) is recently diagnosed with 3C grade 2 serous-papillary ovarian cancer.
initial pet-ct findings
both ovaries seemed to have tumors( ovary size 7.2*4.1*2.8 cm),peritonium and omentum also affected, and very mild uptake in phrenic lymph nodes rest organs normal and shows no evidence of cancer.
treatment= taxol + carboplantin ( 6 doses 450mg carboplantin and 260mg taxol)
CA-125 REPORT
she was given neo-adjuvant chemotherapy
at initial diagnosis= 672U/ml
after first dose= 201 U/ml
after second dose = 30 U/ml
after third dose = 23 U/ml
after laprotomy(surgery)= 9 U/ml
after fourth dose = 7 U/ml
after fifth dose = 4 U/ml
after sixth dose = 2 U/ml
just before the laprotomy a pet ct was done which showed that the omentum is resolved, peritonium is resolved ,phrenic lymph nodes were resolved , all the ascites was completely absent after first dose chemotherapy, but there was some tumor left at the adnexa .
my mother withstanded all the 6 doses of chemotherapy with no side affects(just some loss of hair and mild pain that lasted for only 2-3 days after each chemotherapy) and her vitals are very good( liver function, kidney, heart, pulse)
question 1
what are her chances of totally getting cured? How do you rate her response to chemotherapy?
question 2
does involvement of 1 0r 2 lymph nodes worsen the prognosis substantially?
question 3
what is the standard dose amount for taxol and carboplantin ?
is it 450mg for carboplantin and 260mg for taxol standard dose or her doctor is giving some amount more?
question4
according to the CA-125 normalization and number of organ affected how many years can she expect to live?
question 4
other than grade and confirmation of cancer what importance does the histopathological report carry , what else does an oncologist loo for in the histopathological report ?
question 5
what are the factors does an oncologist take notice before deciding to give MAINTAINENCE DOSE ?
Is it necessary for my mother according to your knowledge ? if yes what should be the time duration and dose for that?
question 6
what is the cause of long term recurrence i,e after 15-16 years or even 30 years is it that some cancer was still left in their body or is their gene code changed and due to which they get expressed again after some years ?
