9 year old son soiling pants

Hello. I am a mom of two great boys- one at college, the other in 4th grade. My 9 year old has been having accidents in his pants for some time now- It is destroying his self esteem. He is a great kid with lots of friends. I worry about him so much. We have to tell him to go sit on the toilet to try to go the bathroom- he usually puts up a fight, saying he doesn't have to go and 9 out of 10 times he goes! Sometimes he goes and it's huge!!! He tells me that he feels it, just doesn't want to stop what he is doing. The other day, he said he held it, stood up, and it just came out of him. He says things like "I hate myself" "I'm an idiot" and it breaks my heart. We went to a counselor- did not help. I feel like I need to do something major NOW before it's too late. I am going to talk to his pediatrician this week. The interesting thing is that if he is sick and has diarrhea- he NEVER has an accident- so, do I give him a laxative on a regular basis to start making him go and make the decision on his own??? What do I do???

Before trying any further interventions, discuss this issue with the MD as a medical condition could be responsible for this behavior.
