What do I need to get ready for delivery?

So I am due in about 5 weeks (June 19th) and was wondering what do I need to get ready to take to the hospital?
I have seen some people take the babys diaper bag and put the baby's first outfit there etc... What did you guys take?

I am due on June 7th, and am still trying to get my bag packed.  So far, I have:

receiving blanket
take home outfit for baby
go home clothes for me (sweatpants, t-shirt, underwear, nursing bra, socks)
nursing pads
toiletries (toothpaste, toothbrush, deoderant, shampoo, conditioner, hairbrush, ponytail holders)

still need to add, slippers, robe, camera, extra batteries, and a book or magazine.

Comfortable pajamas, preferably something that will allow for easy breastfeeding if you choose to do that.  Don't go out and get an expensive pair of jammies...chances are they are gonna get peed on, bled on, puked on, etc....whatever is comfy for YOU...I took sweats and a tshirt.

nursing pads for your bra

lanolin for your nipples

toiletries for showering after delivery

a couple pairs of clean undergarments...bleeding and breast leakage may make you go through more than a pair or two

something to do during labor...puzzle books, a reading book, any kind of craft you like to do, cd's you want to listen to

an outfit to wear home for you-make it something maternity or very stretchy and comfy you won't feel like anything tight afterwards

makeup in case you want to spruce up for pics with baby

a notepad and pen so you can record key things during your labor and afterwards

a phone card (or cell phone) in case you have to call out of local calling area to make announcements

Camera, camcorder, extra batteries or chargers.

for baby..

a bring home outfit (baby can stay in blanket and little shirt hospital gives or you can choose to take extra sleepers or onesies)

some diapers and wipes..hospital will generally provide while you're there but you may need them for the ride home

baby book for recording info and foot prints

pacifier if you choose to use one

receiving blanket

make sure you have the carseat in your car, hospitals aren't allowed to let you leave without first making sure you have a carseat and know how to strap baby in properly
