I didn't have any signs with either of mine. With my first miscarriage in December I went in for my first big prenatal appointment at 11 weeks and the midwife couldn't find baby's heartbeat so they sent me for an ultrasound right away and there was nothing there, just a sac - it's what's called a blighted ovum. I had to have a D&C to basically have my uterus cleaned out, remove the sac and all that.
My second miscarriage last month I'd had an early ultrasound at 7 weeks that showed perfect growth, great heartbeat, everything was just right. And then at 11 weeks we went in for the second ultrasound for sequential screening - and baby had stopped growing, had no heartbeat. We scheduled another D&C but a couple days before the surgery I started spotting, then the night before had intense cramps (worse than labor contractions) and heavy bleeding with clots. The placenta didn't separate entirely, it was an incomplete miscarriage, so I had to have the D&C anyway or else I would've hemorrhaged - the bleeding just wouldn't stop. It was pretty horrible.
So, sometimes you know and sometimes you don't. But it's not worth it to spend your pregnancy worrying about what might happen, you have to enjoy every minute for what it is. It's an amazing journey even if sometimes it's full of heartbreak. One in four pregnancies ends in miscarriage, but staying positive and hoping for the best is all you can do. Good luck.
