For baby: newborn clothes, 3 onsies 1 sleeper , mittens, socks, and whatever else they supply diapers and wipes lotion ect.
For you: two comfy pjs, few pairs of panties, socks, deodorant, toothbrush nd toothpaste, brush , phone charger and anything that will entertain you.

I just finished packing our bags after lots and lots of research, haha. In my sons, I have a going home outfit, a couple blankets ( they where cute and I want to bring them but I really don't need to because hospital supplies them) socks, and hand mittens and a pacifier, and carseat of course. In mine I have hair ties, ChapStick (your lips get super dry and cracked) travel sized hair brush and tolitries, looks fitting pants yoga pants for going home with sweat shirt, bathrobe, nursing bra and three nursing tank tops ( different sizes because I'm not sure how big ill be ) light make up for visitors and pictures, stool softeners( hospital probably provides but just in case ) nipple cream, my own pillow and boppy nursing pillow, light snacks ( after birth you are so so so hungry and it can take While for someone to bring you food and I'm almost evil when I'm hungry so its for everyone's sake. I read many many blogs about what to pack and I took a little information from each one and made a list that I knew would fit me best based on my needs. It seems like a lot but its mostly small stuff and it all fits in one bag, and the carseat is in the car and the two pillows are easy to take along.
Clothes for you and baby hair & tooth brush books magazine journal etc.. Anythin you wanna bring honestly.
