In utero at a 20 week ultrasound, how do you tell the difference between a cardiac rhabdomyoma and an echogenic intracardiac focus?
24 week pregnancy 33 year old women has no other masses are present in any other organs, patient has healthy pregancy but has SUA (2 cord umbilical) and now a only 1 mass found 1mm in left ventricle apex (not on any exterior or anterior walls or septums).
What clues in future screenings would help you sway from one to the other?
Previous pregnancy had two cardiac rhabdomyomas but patients family has no DNA traceable tuber sclerosis -- possible gonadal mutation 1-2% of other sperm/eggs affected.
Screening tests all came back very very low risk for downs, trisonomy, spina bifida etc.
A wrong diagnosis could set this family over the edge or any inaccurate miscalculations. Of course you can't help but note the history and draw a conlcusion of cardiac rhabdomyoma based on this.
Unless tumours are very microscopic at this stage, there is only 1 very very small in the apex....
How would you proceed?
