non cardiac atypical chest pain

Ok.  I am 30 years old.  First I'll say I've had a nuclear stress test, heart CTA, several chest Xray's, an echo,  and a number of blood tests.  

I started having atypical chest pain almost  9 months ago, along with occasional palpitations (early beats),  and lightheaded/weakness episodes.
It's kind of hard to find the right words to describe, it's almost like someone has their hand deep in my central chest above my heart maybe 3 inches below the base of my neck  and is squeezing a large vessel inside.       Often when I first stand up or involve with heavy lifting,  I feel strong pressure in that spot deep in my chest and often I get feelings of cold and/or warmth spreading through my body along with lightheadedness and weakness.      Even at rest I have this heavy tightness in my chest that just feels like something is choking my chest inside.    Not to mention the palpitations that I always feel in this same spot.

Can you please tell me what this can be,  with the normal chest xray, echo, and all?    Should I request a chest CT or something like that?

You might want to ask your doctors if the protocol, 64 slice vs 10 and 16 slice protocols, used for your heart CT angiography (CTA) has any degree of false negatives for the diagnosis of dissecting aortic aneurysm or any other intra-thoracic large blood vessel disease.

Atypical chest pain can also be caused by diseases of the spine and esophagus.  Ask if these have been ruled out.  Ask further if the pain could be a sign of disease of the small coronary artery vasculature.

Cardiac monitoring to rule out serious arrhythmias might be helpful.

Finally such pain can be caused by severe anxiety, with or without panic attacks.

Also...I am very sensitive to even mild tachycardia...it makes me very lightheaded,   and my heart feels like it works too hard especially after a meal.
