First day of treatment and symptoms are MUCH worse! Is this normal?

A few weeks ago had started to get itchy and smelly (Male). Last few days it got very bad. Stopped itching, but was really painful. Swollen foreskin, sores on the head and under foreskin, some discharge. Yesterday went to doctor, it had started hurting to pee by this point. I had also been feeling feverish.

I had a urine/blood test this morning followed by treatment: Ceftriaxone 500mg injection and Azithromycine 1000mg orally one time, and Aciclovir, 5 times a day for 5 days. I have also been taking paracetamol and ibuprofen for the fever.

As the day has gone on my symptoms have definitely gotten worse. The whole head is incredibly painful to the slightest touch, the foreskin/head is much more swollen and there is a lot more discharge. I have no idea what's going on under the foreskin since it's too swollen and painful to pull back. I also haven't gone to the toilet since the urine sample because I know it's going to be agony.

Is this normal? Does this sound like a reaction to the medicine? Or could it be this is the natural progression without treatment and the medicine hasn't kicked in yet?
I also wonder if the crazy swelling is just trapped discharge under the foreskin that won't drain out... is that a thing?

Freaking out a bit. I know these treatments normally take a few days or 1-2 weeks, but I don't know about them getting worse first.
