NGU / Clamydia

I came back to the UK from a trip from the USA recently with a nice new shiny macbook pro (good times) and an itching when I pee'd (bad times).  I had casual safe sex a couple of times while in NYC. I went to the STD clinic in London and they said I had NSU (ie they could see some sort of imflamation cells, but not what was causing it) and treated me with Azithromycin 1g taken as a single dose.  After a week I still had burning when I pee.  So I went back and they took a cell sample from my urethea and said this was positive for Chlamydia.  They also took two urine samples, the first but and middle bit (I had not passed urine for over 3 hours before this sample).  So I was then given a course of Trimethoprim 200mg, 14 pills one every 12 hours.

Today I am about to take the last of those pill's, but i still feel a little soreness and my urethrea.  I have no dishcharge.  Is this normal ? Should I go back to the std clinic ? Or give it another weeks as is the Azithromycin and Trimethoprim still doing their work ? Or should I go back again ?

...go back to the STI clinic and ask them to make a check of your urethra for urethritis. When doing this, they take a sample from your urethra and look for white blood cells under a microscope. In case you are free of urethritis, go to an urologist. If you still have urethritis, you may suffer from NSU related to Mycoplasma or Ureaplasma and get a different treatment regime. Or you may have one of the rare treatment failures of Chlamydia when being treated with Azithromycin.

However, that said, this feeling of persisting irritation lasting for a while may be quite widespread after having had urethritis.

Give it a full 2 weeks after mediation if finished.

OK, I will wait a few weeks, just it still feels a bit hot and burning sensation when I pee. Does it always take this long for Chlamydia to clear up ?  Its just in three weeks time I leave the UK to start a new job abroad, so really want to get this fixed now.

No it can take 1-2 days to start to clear and then be gone within 1 week.

Is it usual to still feel some soreness 2 weeks after I took Azithromycin  and its now 24 hours since i finished taking the Trimethoprim ? My urethra still feels a bit sore.

This sounds a little strange. Azithromycin 1 g is usually highly effective for treating chlamydia. On the other hand, Trimethoprim is neither used for NGU nor for other STDs (only for simple urinary tract infections). This means that they found some other bacteria in your urine. I think you should go back in 2 weeks (dead chlamydia organisms that may provide false positives are usually gone about 3 weeks after treatment) and ask for a total recheck (gonorrhea, chlam, NGU) and ask for explanations for the Trimethoprim (it was trimethoprim, right, not trimetoprim-sulfhamethoxazole?).

Yes it was Trimethoprim.  Actually now you have said that, I think I misunderstood what they told.  So when I first went in they thought it was NGU, and gave me Azithromycin 1 g. A week later after they had analyzed my urine and cell cultures for a week they told me it was Chlamydia.  So I guess the Azithromycin 1 g they gave me should have fixed that, but I still felt some irritation / soreness so I went back and they gave me Trimethoprim as they said I had urinary tract infection (they looked at a some cells under a microscope to conclude this).

I have finished the Trimethoprim, but i still feel the same, ie some irritation.  So I guess I should go back, maybe they will refer me to a urologist ?  I am just worried I have something else going on, thats causing the urinary tract infection.

...you should not assume that people in this forum are medical doctors. Most of us are only well-informed people willing to share some of our knowledge. If you really look for the most exact and updated advice available, pay USD 15 for getting the advice from Dr. Hook or Dr. Handsfield at the Doctor´s STDs forum. They are among the leading researchers in the field of STIs in the world. That said, I guess the people in your own STI clinic are just as good at dealing with your seemingly minor problem.

Thanks Lars for the advice.  I went back the clinic 4 days ago, they said the the urine and cell sample they took a week earlier showed no signs of any infection.  They also said it was normal to feel soreness for a few weeks after being treated.  The doctor examined my urethra and said it looked the normal colour on the tip and looking inside the urethra also.  So he said the discomfort should decrease, which it does seem to be.  So looks like I am very much on the mend.
