tests done

Hello Doctors:
Sorry about my terrible english, i don´t practice it since a log time, however i´m going to try to make my question in a good way.
My risk is vaginal sex using a condom with a striper. I took my condom and after a while tested it with water. I saw a small hole on it, the size was minimal, nearly as a needle. It was impossible to notice it without the water test. Maybe i broke it on my pocket. The question is that i´m not sure.
7,5 days after, I made a few test, that laboratory cosidered conclusives:
1.- PCR RNA.- VIH 1.
2.- PCR for SiphILIS.
3.- PCR DNA HEPATITIS B.
4.- vih 1/2 and p-24 4th generation (duo test).
5.- exudation for clamidya Gonorrhoea and others.
Do I need any father tests? I am also scared about vih2. Thank you Doctor
I know my risk is low but i am scared.

Hello Doctor: As you advised me I finally did yor test after 4 weeks, exactly 30 days after exposure. These are all my resuslts:

After a week:

1.- PCR RNA.- VIH 1.- negative
2.- PCR for SiphILIS.- negative
3.- PCR DNA HEPATITIS B.- negative
4.- vih 1/2 and p-24 4th generation (duo test). negative
5.- exudation for clamidya Gonorrhoea and others. negative

After 20-21 days:

PCR RNA VIH.- NEGATIVE
- PCR DNA HEP B.- NEGATIVE
- PCR SYPHILIS.- NEGATIVE.
- SWAB FOR CULTURE AND ANTIBODY FOR Chlamydia AND OTHERS.- NEGATIVE.
- Hep BsAg, and Syphilis serology.- NEGATIVE
- VIH 1/2 AND ANTIGEN P-24.- FOURTH GENERATION.- NEGATIVE.
- HEMOGRAM FOR LIVER.- FINE AND NORMAL.

After 4 weeks I did all tests you recomended me:

- HIV 1 and 2 Ab+ p24 Ag.- Negative.
-Hep BsAg.- Negative.
- Syphilys serology.- Negative

Can i believe my results now? thank you Doctor.

Good evening Doctor:

As I told you on my last post, i was going to inform you about my results made 3 weeks after exposure. I know, you told me to re-test at 4 weeks mark for antibodies, but I live far away from the city and I don´t frecuently go there. Sorry. That´s the main reason I took PCR TESTS once more. For being completly sure and in order to the advice made by my laboratory (I was still on window period).

These are my retest results 20-21 days after exposure, which laboratory consider conclusives with the other tests made before (7 days from exposure):

- PCR RNA VIH.- NEGATIVE
- PCR DNA HEP B.- NEGATIVE
- PCR SYPHILIS.- NEGATIVE.
- SWAB FOR CULTURE AND ANTIBODY FOR Chlamydia AND OTHERS.- NEGATIVE.
- Hep BsAg, and Syphilis serology.- NEGATIVE
- VIH 1/2 AND ANTIGEN P-24.- FOURTH GENERATION.- NEGATIVE.
- HEMOGRAM FOR LIVER.- FINE AND NORMAL.

Before i made these tests i read a post from Doctor Hunter who consider that combination of PCR TEST AND ANTIBODY together is nearly 100% secure for vih. It is curious because he is not a defender of this technology in order to false positives and cost. He Also says that after 10-12 days is nearly 100% secure.

I´m not sure because I don´t speak english correctly, but Freedom Health also support tests done at 7 days mark, such as my laboratory and others do specially with vih, Syphilis and HEP C.

For Chlamydia/Gonorrhoea, I think they did not used PCR they have another technologies which they applied onto swabs. They told me a week was enough, but i decided to repeat it as you told me.

My Laborary agree with FREEDOM HEALTH, that for hep B everybody have to wait at least 20 days for PCR DNA TESTS.

Hola buenas encontré su pagina por casualidad y me ha dado mucha alegría que haya gente que haga esta labor tan buena.  Mi caso es el siguiente:
Hoy me acabo de hacer una prueba rápida de detención de anticuerpos anti VIH- 1/2 (oral quick advance). Y el resultado ha sido " No reactivo"; osea no ha detectado el virus. Mis dudas son las siguientes:
     1- He tenido dos practicas de riesgo ( penetración anal sin preservativo, sin eyaculación y siendo yo siempre el que penetra). Del primer caso han pasado ya 5 meses. ¿ Me certifica este test el 100 % de efectividad para este caso en concreto, o debería hacerme un anilisis sanguineo en el medico para que sea mas seguro?

     2- Del segundo caso sin embargo han pasado solo 73 dias, en este caso fue igual que en los anteriores ( sin preservativo, anal, yo el actvo y sin eyaculacion) (en este caso el chico me ha confirmado que tiene VIH). Me han dicho que me lo vuelva a repetir dentro de 20 dias cuando pase el periodo ventana. ¿ Que probabilidad hay de que me haya contagiado? / ¿Significa que tengo menos probalididad por el echo de que solo falten 20 dias? ¿ Mejor siempre repetirsela una vez hayan transcurrido mas de los tres meses ( esperar  un par de meses más por si acaso) y por el medico ( analisis de sangre) ? y asi adegurarme de que todo está bien.

Good Morning Doctor. I have to go to the city for work this week on wednesday, and i´m going to go to test for Chamydia/gonorrhoea and others. I dont usually go to the city,  i live in a small town with no laboratoy.
In the laboratory i mentioned before i´m going to repeat all the test i did before because i´m not still on 4 weeks mark and they use PCR TESt which they mention are absolutly conclusive. I will be on 3 weeks and 1 day mark.
I´ll do the following tests that they have recomended me because of my fears, as you know they told me all made before was conclusive, and they are speciallist on this PCR tests:

- PCR RNA VIH.
-DUO TEST AND P-24.
- PCR DNA HEPATITIS B (they agree with you that this test must be done later, and 3 weeks is more than enough.
- PCR SIPHILIS.
- ANTIGEN AUSTRALIA AND ANTIBODIES
- Swab, culture  for Bacterials, hongus and test of clamidia and Gonorrea antibodies. In this matter they dont have pcr i think, they use another technologies.

I hope this will be conclusive. They have also told me that if i am scared about vih 2 which is very uncommon i should do a vih 2 test on sex weeks at least. They dont have antigens AB for vih 2, and believe this is not necessary.

If you have any idea or desagree with theirs recommendations i would like to hear them. Thank Doctor.

I agree that the risk is minimal; I would sya practically zero.  Also when condoms break, they usually rip, and not have holes.  However if you want to be completely sure, I would advise you to have the following tests and these times:
- Chlamydia/Gonorrhoea PCR test after 2 weeks
- Blood test for HIV Duo (HIV-1 and 2 Ab + p24 Ag), Hep BsAg, and Syphilis serology, after 4 weeks.
These would be final and fully conclusive at those window periods.

I know my risk is very low but i don´t want to infect anybody, thats why i want to be plenty sure.

Some doctors as i read in internet says that is imposible to find a minimal hole in a condom during sex because condoms break completly and every body would notice that. Well, i found one very very very very small but it was there.

After your answer, i went to laboratory and explained that i wanted to retest in two weeks. The fact is, They insist tests are absolutly conclusive with PCR and uretral swab test (no PCR I THINK). It dosen´t matter, I will follow your advise.

When you mean " to check up at 4 weeks" is for all the matters (vih, hep b, gonhorrea, Clamidya and Siphilis...". i´ve seem that you defend this kind of tests (pcr) from 7 days mark but also recomend sometimes a 4 week retest for vih duo and p-24, in high risk.

What shall i do with the others contagius diseases, is the same time, 4 weeks?

They have told me that if i want to repeat i should do an australian antigen, a vih duo for vih2 4th generation, because they dismiss anything else. Do you agree with them?

They say Siphiis (PCR), Clamidia, Gonhorrea (uretral exudation) is conclusive.

Thank you Doctor and sorry for making another question, but nobody have the same opinion and i´m getting mad.

Hello,
I believe that your risk was quite low in the first place.  However if in doubt, it is always best to be sure and get a full STD screening.  All the negative tests that you have had are highly reassuring.  However because they were performed only 7 days after the sexual encounter, they cannot be considered fully conclusive and I would advice you to get a check-up at 4 weeks.

The size was less than a millimeter or as a point of a needle absolutly minimal.
