Help! Prostatitis,GNU, STD or just in my mind?

Four years ago I had brief, unprotected, semi-erect vaginal penetration and non-ejaculatory oral sex with a female college student. We were very, very drunk. Two days later during intercourse with my spouse I had a slight discomfort upon ejaculation. I didn't consider the prior encounter with the co-ed and figured it was because I had eaten a very hot habanero sauce the night before. However, I had a slight, not painful, tingle in my urethra for weeks after with urge to urinate. No discharge, no pain on urination (actually felt a little better when i did). Became very concerned (and focused on the possibility of an STD) and went to family doctor. She referred to urologist. "Dr. Johnson" examined me and diagnosed it as most likely Prostatitis. Did not mention the Co-ed to either doctor. No STD test. Urine test, for infection, came back fine. He told me to scale back on hot food and coffee and to not wear ribbed underwear. I did and symptoms went away as he said they would a week or so later.

Masturbated or had unprotected sex (with my wife) nearly once a day for two years without issues or concerns. However, a few times if I did not ejaculate for a number of days, I would have a slight burn upon ejaculation (still felt great, just a slight residual burn that went a way a few minutes later). Not painful and no "tingle" afterwards.

2 years ago I, again, had brief, unprotected, vaginal penetration with a female, who is not my wife. Along with a soapy hand job. No ejaculation. Less than a day later the "tingle" was back. Went away a week or so later. Continued to have sex with spouse with no issues. Again, no discharge or painful urination.

Last week, I had 3/4 erect, unprotected sex with an older woman in a committed relationship. Had a child a few years ago. Claims to have never had sex outside of her marriage. Uses condoms with her husband. The whole time was thinking about the return of the "tingle". 12 hours later, while wearing ribbed underwear and after eating a VERY spicy lunch the previous day, I started to feel the symptoms return. Had fully erect, unprotected sex with the same woman that night. The next morning the "tingle" and urge to urinate were back. I still have to tingle to today. No pain (urination or ejaculation), just a "tingle" in the head.

Once last point... my semen (no sperm after vasectomy) seems very stringy when I have the "tingle," but not when I don't.

So, the questions is... Is this an STD? Prostatitis? GNU? Or, just my mind reacting to these affairs?

Stop having sex with people other than your wife!!! Dont you realize that you are putting her at risk??!!! You dont deserve her!!! It can take up to six months for some stds to diagnose! You both might be infected!! Stop being a coward and man up- be honest with her and your doctors!! Many stds dont even have symptoms!! You trust the woman who cheats on her husband??!!!!

Your symptoms do not sound like a STD. Both doctors in the STD expert forum describe something that is described as "genitally-focused anxiety", wherein men often have a heightened perception of sensations in their penis after sexual encounters where they feel regret/guilt/anxiety etc. Given the lack of clear STD symptoms and the consistent pattern (i.e. occurrence after affairs) I suggest that is the most likely cause.
