STD chance

Good morning,

Thank you for providing this community and resource. I recently had unprotected sex with a friend of mine on 12/5. We have been fwb for years, but hadn't had sex in a while. We are both in our mid 30's. I'm male and she's female. She was tested for STD's from her gyno in 9/22 and 4/23, all negative. She had not had another sex partner since 5 weeks before that first test and she has never lied to me. Chlamydia and gonn were tested by urine. After sex I felt fine, but noticed an increase in urination frequency. This is usually stemming from coffee intake. I told her about this and she went to get tested again for assurance by her gyno. This was over a month after sex. All came back negative. I went to my PCP in February and they ran a urinalysis and found no issue, but I had peed within an hour before this sample. I was referred to a urologist, but have not had a chance to go yet. It was more the check my prostate, though bloodwork showed no problems with kidneys or prostate.

My urine stream is fine, but towards it the end it is much slower and dribbles. I kept looking inside my penis to see if there was anything and I think I aggravated it. I noticed a clear discharge after peeing and pooping, but it doesn't just ooze out constantly. Nothing noticeable on my underwear ever. I've read this is fine. I do have back pain (not unusual for me), sometimes penis tip irritation (especially after running), and pain down my left leg, thigh, foot, groin and butt which sometimes feels like testicle pain?

Am I freaking out too much? What are my real chances of having an STD if she has tested negative 3 separate times. I know you're going to tell me to get a test to confirm, but I'm just looking for piece of mind.

Why haven't you tested specifically for STIs?

Honestly, it sounds like you have a prostate issue, and not an STI, but only a doctor who examines you can tell you that. The dribbling, slower stream, tip irritation, and increased symptoms with caffeine all suggest a prostate issue, but the urologist will be able to help with that. You should make time for that.

So yes, your risk sounds really low because your most current partner is negative, but without knowing if you've had other partners, I can't say for sure. As you knew I'd say, test, and call the urologist.

No other partners. Testing always gave me anxiety so I don't usually put myself in a situation where I need to. Could it be Mgen? Would prostate issues clear on their own without treatment of antibiotics?

Can I take my most recent partners test and the two before as conclusive that she in fact does not have an STD? I've heard of false negatives and urinating too soon before tests altering results. Would being sick with other illnesses (colds, pink eye, fatigue) not be contributing symptoms of any STDs?

Mgen - maybe? Unlikely. You don't have any sign of infection, like white blood cells in your urine.

There are several different types of prostate issues. I can't begin to tell you if yours will go away without treatment. Some are chronic and non-bacterial, some need antibiotics, some are far more serious (quite, quite unlikely at your young age). You need to see a doctor.

Yes, I would conclude that your partner does not have the STIs she tested for. I don't know what was included on her testing, but the ones that would cause symptoms similar to what you are experiencing would most likely be included.

Did you have the cold/pink eye/fatigue? I ask because pink eye is being linked to covid now, and there is some evidence that covid can infect the lower respiratory tract in males.

So what's up with the testing anxiety and seemingly not wanting to go to the doctor? Doctor anxiety? Lack of insurance? Worried about a regular partner finding out?

Again, it really, really doesn't sound like an STI to me. It sounds prostate. You'll do yourself a disservice if you don't follow up on this. If you don't have insurance, call your local health department and ask what services they have for people who are un- or underinsured.

How recently had you urinated? Usually, with the hour time frame, it's to make sure you haven't flushed out bacteria and have enough to show on a test, not white blood cells.

Make the appt with the urologist now. If you're in the US, there will likely be a wait. You don't want to put it off and then end up not getting seen for 6 or 8 months, and have things just get worse.

Since you mention the increase in symptoms with caffeine, decrease that while you wait. Alcohol can also increase symptoms, so watch that if you drink alcohol. Increase water. Those things can really help symptoms if it's a chronic, non-bacterial prostatitis.

I've combined some of your questions since they go together.

It would be constant. Discharge after urinating and pooping is associated with the prostate. It doesn't always indicate an issue when seen on it's own, but combined with your other symptoms, it does.

* Would it be constantly staining underwear white and yellow? have color and odor?

Discharge color on underwear often depends on the color of your underwear. If it's yellow on white underwear, it may indicate an infection. It it's white, probably not. Clear or milky white discharge is most often normal, depending on when it happens. Any other color may indicate an infection.

* And would it be accompanied by bad pains when urinating?

Pain with urinating is not limited to an STI. It can be from a urinary tract infection, a prostate issue, a physical issue like a narrowing of your urethra, or an STI, among many other things, including an STI.

* I forgot to mention your pain down your leg that feels like it's coming from your testicle. The pudendal nerve may be inflamed or irritated if the prostate is inflamed. It could also be caused by epididymitis or orchitis, but you don't describe the swelling or amount of pain that those cause. This pain into the leg is never caused by an STI.

You need to see a doctor, sooner rather than later. If this is bacterial, it is only going to get worse. I understand doctor anxiety, but that anxiety won't get any better the longer you wait. Do you have someone who can go with you? A friend, sibling, parent? Maybe that would help.
