Understanding Herpes Test Results

Hello,

I took a herpes blood test at my OB/GYN's office while I was there on October 17th for my annual pap smear.  I got a call Tuesday night from my OB saying I needed to come in to hi office because my pap was abnormal.  The pap shows that I have HPV - High Risk Positive.  My OB was out of the office so another doctor read these results to me.  When I asked her about my HSV I and II test she walked over to theri in-house Labcorp worker who drew my blood and asked her to print out my results.  My results are as follows:

HSV 1 IgG, Type Specific            <0.9                          index   0.0-0.8
                                                           Negative     <0.0
                                                           Equivocal    0.9-1.0
                                                           Positive       >1.0

HSV II IgG Type Specific      1.88 or 1.8M (print out hard ot read on the second number) index 0.0 -0.8
                                                            Negative   <0.9
                                                             Equivocal  0.9 - 1.0
                                                             Positive > or =1.1

HSV IgM I/II Combination          <0.91       Ratio                 0.00 - 0.90
                                                            Negative <0.91
                                                            Equivocal 0.91 -1.09
                                                            Positive >1.09

The doctor who read my results said I was negative for HSV I ( I think I may have had one cold sore on my lip when I was 13 but that's it) and she said that I was positive for HSV II because at some point in the past I was exposed to genital herpes.  I have never had any reason to think that I have this disease.  No breakouts, sores, etc.  I don't know what type pf test they used but I want to know in your opinion could this be a false positive or should I accept these results?

Thank You

I just left a new PCP's office and he ordered three new blood test to be done at Quest.  One is a Herpes Simplex Virus 1 and 2 PCR 42887N, another IgG and an IgM which I know is useless from what I have read.  He also said that an HSV 2 Igm of 1.8 is not low.  This is contrary to what I have read on this and other websites.  I am afraid that I am getting misinformation so I made an appt with an Infectious Disease specialist for next week.  Have you heard of the PCR test?  Should I hold off on getting another test done?

Yes it can be but it's well worth following up on it when you can't recall any previous symptoms just to err on the side of caution.  It would be cruelly ironic if you really didn't have hsv2 but got involved with someone else who had hsv2, thought you didn't have to take precautions and ended up with hsv2 and obvious recurrences on a regular basis don't you think?  Not that genital herpes is the end of the world by no means - I've lived with it myself for 21 years - but if you can avoid it - why not.

One final question.  If a new test comes back with low HSV should I aceppt the results?  Can HSV be low, positive and accurate?

They can certainly take the time to contact the makers of the test and talk to them about this too. They are quite open about the incidence of false positives and will gladly talk to your doctors about it in more detail.  Considering that not all lab corp labs use the herpes select - the incidence of false positives is even higher in some of the other tests :(

I know it's hard to imagine that you could be infected with hsv2 for years and never know it but it's more common than folks ever suspect.  Most folks think that if you had genital herpes you'd have obvious and painful blisters that reoccured frequently. It's much more likely to present with mild or even vague symptoms like itching, redness, discharge etc or be misdiagnosed as something else ( uti, yeast infection, hemorrhoids, shaving rash etc ).  Hopefully within a few weeks you will have this all straightened out and know what is going on.  You might even want your doctors to order a herpes WB from the U of washington as your next test.

Thank you Grace,

My OB/Gyn (or the doctor that I actually saw who read the results) and my PCP are farily certain that any new test would be positive but I want to be retested asap.  I had unprotected sex July 4, 2006.  Would the result still be this low aftewr a year and a half?  The OB said it appears that I was exposed to the virus years ago possibly even when I lost my virginity.  I'm 34 years old and can't imagine that this has been in my system all of this time.

Your hsv2 result is still fairly low.  It would be worth repeating the igg hsv blood test in another month or so and seeing what their results are. Another option would be to see if anyone offers the biokit hsv2 blood test near you and get that done in another month ( it only tests for hsv2 and you get your results back in 20 minutes ).

Fingers crossed that your follow up pap testings are all negative and that your hpv isn't much of an issue :)
