dry flaking skin on the shaft of my penis

For several years I have had a condition in which the skin on the shaft of my penis and sometimes on the head and rim, but mostly the shaft. when I put water or saliva on it it seems to go away for a few hours then it comes back. some times it is big thick flakes other times they are small thin flakes. I have tried lotion on several occasions but it always returns. can anyone tell me what this is and how to get rid of it?
