Path Report INTERP??

Hi Dr.
I have tried to get here to this board for a while...

1. Right Lobe Throectomy
A Papillary Carcinoma
B Tumor is 2.2 CM in hte greateset Dimenesion
D. Tumour is not is seen int he auctual inked margins, but exceeeds widly 1/10 of 1 millimeter of the inked margin
E. 3 of 4 Lymph nodes contain Metastasis Papillary Carcinoma
F. PATH STAGE 3, N1A, MX. Stage group one

2. Left lobe Subtotal Resction A typical Follicular proliferation, 6mm in diameter not seen in margins

RIGHT LOBE
The Tumor does not eshibit widsepread extrathyroid extension. Tumor extends focally throught hte caspsul of the thyroid, but not extesnivly into, adjacent fatty tissues. It is not seen to invade muscle or neck tissue

LEFT LOBE
The left thyroid eshibits some suspious but not definative for follicular varient of pailllary carcinoma. It does not exhibit classic architectural and cytologic features for follicular vairient of Papillary Carcinoma. seen in hte right lobo/ Follecular adenoma cannot be excluded/ However in the malignant process of the right lobe full removal of the rest is reccomended..

THis report is is a BAD prognosis?

Chit Chat Nine and Utah Momma told me to get this posted it took a bit but here I am....

I am seeing a Utah Huntsman Cnacer Doc TOMORROW!!! My Surgone theone that did all this wants  to take out the rest of my thyroid WED this week!!! Then follow with RAI. HE only left 10% of the thyroid in. Any advice or expertise would be so appreciated.

I know I have cancer but how many types??

You have minimal thyroid extension (hence the T3 and not T2) with some lymph node involvement (not uncommon in papillary cancer).  The prognosis is still excellent.  Would have the slides reviewed by an expert pathologist to make sure there is not any aggressive subtype.

Before more surgery, you need to have a good neck ultrasound to make sure there aren't other lymph nodes.  That would change the extent of surgery.  most people do not know how to do these ultrasounds -- but in Utah, would go see Dr. Dev Abraham at the university in Salt Lake City -- if that is an option.

If not, then at least a CT of the neck to look for large lymph nodes. before the completion thyroidectomy (although from the comments it seems like they removed the right and left lobes already).

Then you will need I-131 treatment -- ask about the option of using thyrogen rather than coming off levothyroxine.

I agree as well! He is VERY smart and I love he tell you wah tis going on and lie in the weeds behind you back. I can take wehat ever just let me know, Ya know!?? He is great!!!!

Wonderful advice from Dr. Lupo ... keep us posted on what your 2nd opinion says.  Yes, those tests are so important .. I would have them done before the surgery even if that means postponing the surgery for another week if needed *but I hope not*.  I wish Dr.Lupo lived closer to me .... yeah, I'd go to him for my ongoing Thyroid Cancer care!

I do not take ANY MEDS.... He said I did not need any now.  I am 9 days post Surgery, of my thyroid on the left lob in there for some odd reason??? I hope this new Dr. will shed some light on this deal. What is 1-131 treatment?? Waht do you think about the RAI treatment? I printed your mail so I can talk real good to the new one!!! Dr Brandon Buntz. ENT specializes in cancer as well.

RodeoQueen comes to you at our request on Thyroid Disorders as she has upcoming surgery and has cancer that looks to have spread, etc.

A few members (inc. myself) felt your input may really be able to help her with her cancer.
