large nodule with pain

thyroid nodule 5.2 x 2.6 x 2.6cm i had for about 7yrs dr been watching it.  it started out  2.5cm  on synthroid also had I 131 the ultrasound report was diffusely inhomogenous unable to identify definite normal appearing thyroid tissue.  may also represent diffuse involement by numerous subcentimeter adenomatous nodules. i know its a complex, cold nodule. we're doing another fna this wk.  it was 4.8 x 2.4 x 3.5cm before I 131.  i have pain in ear,neck sore throat and hoarseness.  they say its goinf to be risky cuz of prior thy surgery 17 yrs ago.  that there's scar tissue in the area, afraid of vocal cords??  tsh 30.2 now down to 8.4  on armour . should we keep watching the nodule just really confused.  thank you!!!!!

SEEN DR FOR FNA HE WAS SURPRISED HOW BIG IT IS AND SAID ITS GROWING BACKWARD (DEPTH) ? SAID NEED TO HAVE SURGERY.  FEELING LOTS OF PRESSURE ON NECK,EAR PAIN HEAVENESS IF CHEST.  I SOMETIMES THINK I"M LOSING IT.  REALLY NERVES EVEN THOUGH HAD THY. SURGERY 18 YRS AGO.  THANKS  EVERYONE FOR THIS SITE I'VE LEARNED LOTS.

this is a fairly large nodule - if there is a significant amount of cystic fluid, this could be drained and may alleviate some of the symptoms.

Repeat FNA is a good idea, if this is not clearly benign, then surgery.

May end up needing surgery -- there should not be significant increased risks with an experienced surgeon -- you may want to check out university hospitals, etc around your area for a thyroid surgeon to discuss this with.

TSh needs to be closer to 1.0
