homogenous versus heterogenous

what does it mean had ultra sound right lob 4.5 x 2 x 1.5 cm, left 4.6 x 4.5 x 1.4, isthmus 4mm
there is geographic heterogenous pattern with in thyroid, no defined nodules. I know the thyroid is enlarged, and had uptake and have graves, but what does geographic heterogenous pattern mean?

not sure about "geographic" -- but heterogeneous means inflammation usually and that would be consistent with history of Graves.  The Left lobe measurements are unusual -- did you mean 4.6x 1.5 x1.4 ?  if so, this is not significant enlargement by ultrasound criteria.
