groin pain and other problems

my issue starts 12 weeks ago. I had protected sex with a new partner and unprotected oral.
im not sure if my symptoms are related to this encounter but thought I should mention it all the same.

im 28 years old and healthy otherwise

first of all.. the reason im asking on here is that Im in the middle of the sea on an iol rig and cant get to the doctors for around 10 days.

Around 10 weeks ago I started having mild pain in my testis, it would come and go in waves. I went to the doctor and signalled him to a few lumps i noticed in there at the same time.. he said they were normal and sounds like iv had a little bit of trauma at some point and to try using suportive underwear.

the next day I had sever pain in my lower abdomen (groin area) and so went to the emergency room worried that I could have appendicitus.

they kept me in for four days and done some tests... Im not sure exactly what they did test for but they took two lots of urine and quite a few blood samples... I know one of them was for a blood culture...they also did an ultrasound of the general area but found nothing.   they released me saying that they thought I probably had some tendinitus (groin strain) and that I should rest and take diclofenic.

a few weeks later the pain started to spread to my back and to the other side in a similar place... its not a severe pain anymore but more of a constant ache.

in the last 10 days or so... the tip of my penis has become very red and is irritable..  it is not sore to pee but i seem to need to go an awful lot.

Im not sure if im having a discharge because sometimes im wet down there but it seems more like a small release of urine... i try to catch it when I feel it about to come out (rush to the toilet before my underwear soaks it up) just to see if I can get a better idea of its consistentcy but I think I had that sort of release before anyway (i have for quite a long time had a problem with needing to pee a lot and completely emptying)

My anus is slightly irritavble at somepoints but not all the time.

I was fevered when I went into hospital... I was also fevered yesterday (37.4C) and think I am today too.

could anyone shed any light on what this could be..

at first I thought it must be an STD but after all the research I have found that its unlikely given my encounter... also the hospital would have noticed something to indicate that.

at the moment im thinking its maybe a prostate problem but not sure if im basing that on enough.
have also noticed a tiny black thing underneath my foreskin ... its like a grain of salt or sand but black...   could be some fluff from my underwear...  seemed hard... have observed this twice.

so to recap

groin pain on both sides of groin... always there but gets worse and better (waves)
also a lot of pain into my pelvis
Pain in testis
sometimes pain around my lower back at the sides
red inflamed tip of penis (not pain to pee just irritable)
Fever, high temp
no nausia
all the pain is more predominent on the right side
stool seems normal but sometimes irritable
tip of penis feels sore most of the time... but very mild

any help would be great....  dont want to get sent home after keeling over or anything! .

The liquid could be your natural lubrication.  As I am not a man, I do not know whether this is normal or not.  But having a white, yellow or foul smelling discharge in both men and women is not normal.

I do not know whether it is rare or not for a young man to get a UTI, but in any event I do agree with your doctor that you do need further investigation to identify the cause.  Especially so if it does not clear up after the antibiotics, or settles down and flares up again.

The problem may be from the urinary tract, or a problem with the prostate gland.  Any blood detected by a urine test in a male should always be investigated no matter what age they are.

You will have to see your usual doctor when you get back on the mainland.  Keep a record of all your symptoms and your medication for future reference.

yeah it seems to have settled down (the weather)

ok will give it a few days and will let you know..

didnt think a fungul infection could cause liquid in urethra...

on closer inspection i think the liquid is actually clear...cant really "milk" any out but i think it was the light i was using making it look white... is it normal to have fluid in there anyway?

just feel quite wet down there... maybe my imagination!


the doctor said it is extremely rare for a young man to get a urinary infection so the underlying causes need to be identified

Glad you went to see the medic.  It's about time you had your urine tested.

Leucocytes present in the urine is an indication of an infection.  The blood could be from the urinary tract or from your prostate.

5 days of Trimthoprim is usually what is given out for a urinary tract infection.  I do not think that you have an STD.  You will have to get checked out for that when you get on the mainland.  I am not sure which antibiotic is used for STD, it could be erythromycin or a different antibiotic.  

The whitish fluid in the urethra opening and your sore penis is likely to be a fungal infection.

After you have finished the antibiotic, get your urine checked again to make sure it is clear.  If you are still sore ask the medic to give a tube of Canesten cream or Fluconazole which is a single dose oral capsule (if they have any).  A doctor can prescribe the cream and the capsule, and they can also be purchased from a chemist without a prescription.  

Make sure that you keep drinking and do not get dehydrated.

You should start to feel better after 2 or 3 days, but make sure that you complete the whole course of antibiotics as prescribed.

hi again jemma..

I have seen the medic now..    she tested my urine which showed up blood and leucoucytes...  

she thinks I could have a hernia but she isnt sure..

she phoned the doctor who told her to give me 5 days of trimithoprim (400mg per day)

she thinks its a urinary infection...

so what do you think...  would this antibiotic cure a uti in five days?..  also.. if its an std would it clear it?

I have noticed some whiteish fluid when I look inside my urethra opening???...

It's got to be live natural yogurt, not the sweet variety.  It best applied at night time when you can put a lot on.  

Don't wash with soap or shower gel.

If you have a hernia you will have to be careful and not strain.
You do need to be checked out.  How's the medic?

ive been ok...  just really sore

yogurt doesnt seem to be making a difference..

pretty sure there is a hernia there now

Scotland always has the worst weather.  
There has been floods in Lincolnshire and some bungalows have slid down to the cliff by the sea in Norfolk.

Hope you wife has plenty of food in the house and doesn't have to go out in this bad weather.

The winds have settled down in the Midlands we now have rain.

How have you been?

Oh no!  LOL - I just can't believe a medic on a oil rig is off ill with sea sickness.  You'd think he/she would know what to take for it.

The weather is pretty bad on the UK mainland too.
Scotland is worst affected, and a town in Norfolk has been evacuated.
I am in the UK Midlands, tiles have fallen off some house roofs,  branches and tree trunks falling about.  Hubby had a branch hit his car twice tonight and came home a little shook up.    It has damaged one of our fence panels too.  

Hope the weather settles down soon and that you are all safe.

When you were in hospital with a suspected appendicitis, it is very likely that your mesenteric glands were inflamed.  This can mimic appendicitis.

If you are worried you should get your medic to examine you.

Hernias pop out when coughing.  Have you been moving anything heaving?
The most likely thing it could be is a ganglion.  These are little lump of grizzle that can appear anywhere on the body.  These are like little lumps that you can move; if it presses on nerves it can feel painful.  The other thing it may be is a swollen lymph gland.

Hernias, ganglion and swollen lymph glands would not give you a red penis.
A hernia and  a ganglion would not give you a urinary tract infection.

If it is your gland that is swollen, that could mean that you have a viral or bacterial infection.

Did you not go to see the medic and get your urine tested?

hi jemma..

ok today I have noticed something else, there is a small bulge in my groin around a half inch up and to the left of the base of my penis... when I push on it, it feels like a small bit of muscle and moves like a hard bubble with a weird sensation (not painful) ... when I push down on it and cough it seems to contract.

Is this a hernia?

if it is then could it be possible that it is causing a lot of my problems i.e could a hernia cause a bladder infection or uti, red tip of penis etc..?

getting a little worried about it because its not happening where the pain is and I remember when my friend had a hernia he was told if it is a generalized pain then it is likely being stangulated...

any ideas?

Canesten cream will be fine.

The urine dip sticks will detect leucocytes, protein, nitrite, blood, sugar levels (if these are over 11).  Some sticks have more info, some sticks less.
I should imagine that you medic urine test strips will have the ones I mentioned above.

Leucocytes, protein and nitrite indicate an infection.  Nitrite is specific to bacteria in the bladder called Cystitis.

Blood indicated in the urine as well as leucocytes and protein is a urinary tract infection.  Blood only in the urine in a man should be investigated further as this may be from problems with the prostate gland.

In all cases of any urinary problems it is important to drink fluids (not alcohol).  No more than 2-3 litres.

When you are on the main land I would still get checked out for STD to be on the safe side and to put you mind at rest.  As well as getting your waterworks checked out if there is no improvement.

have just heard back...

she has had 2 full sexhealth examinations in past 2 months... including oral swabs (she went a second time last week after I told her I had symptoms)

everything came back neg both times

so does this mean I can completely rule out std from this encounter?

obviously il still be sure to get tested but should expect neg results from it.

hi again,

the medic cant do anything today as she is due off... the new medic is on tomorrow and im going to get him to test my urine with a dip stick... what things can be picked up by this... if there was std or other infection would something show up?

The person I was with has told me she has been tested negative for sti/d's and now I have asked her if they did any throat swabs so I am waiting to hear back from her so that should clear that up...   (sti's are what i am really worried about)

I will keep you posted on how I get on

Candida does cause Thrush symptoms and the red tip can be from a fungal infection.
So the natural yogurt applied to your penis will help.

Does the medic have antibiotics and penicillin on the rig?  I am sure she must have some urine reagent sticks.  These are dipped into the urine and can indicate Cystitis and other problems like infections and blood in the urine.

UTI means urinary tract infection.  A urinary tract infection means an infection anywhere along the urinary tract which is kidneys, ureter, bladder, urethra.  That is why it is important to drink and not get dehydrated.

If you are worried that you may have an STD you are going to get tested when you on the mainland.

With an viral or bacterial infection your temperature will go up and down.

ok thats good to know..  I read that UTI in men is very very rare?

could it be more likely an sti/d with white bits in urine and red tip?

will go and get some cranberry now... that is a good idea!

paracetamol isnt taking the fever down so hopefully it wont get worse.

no we just have a medic on board... shes more like army medic type thing.

could candida be a possibility?

Probably a urinary tract infection with the white bits in the urine.
With the sore throat as well it is likely to be a bacterial infection and you may need an antibiotic.

The only way you would find out for definite is by having a urine sample tested.

Just to let you know that taking antibiotics can bring about having a fungal problem, because the antibiotic destroys the good bacteria as well as the bad and Thrush can take a hold.

Make sure that you keep drinking, water is best.  If you do start to feel uncomfortable peeing, aske the canteen staff if they have any Bicarbonate of Soda.  Using a teaspoonful in a glassful of water will help to ease any irritation by neutralising the urine.  You can do this up to 3 times only with a hour between each dose.  

If the canteen staff have cranberry juice, that will also help by stopping the bacteria clinging to the bladder.

It is a shame that you have no access to a doctor.  Is there no doctor on the rig at all?

ok jemma thanks I will definately have a look...

today I have noticed that every time i go to pee..  (which is  a lot)

there is white particles in the urine...  its new... iv never had that before... its like cloudy with white bits in it..


would that indicate something in particular with my current symptoms?

No it does not always hurt when you pee.  Sometimes it feels like a burning sensation, this depends on how acidic the urine is.

Having a bladder infection can cause a pulling down sensation in the lower belly sometimes.

With a urine infection, you would be wanting to pee more as well.

Look up Balanitis on the web, this explains what can cause redness at the tip of the penis.

It seems that you have either a bacterial or viral infection and possibly a fungal infection on your penis.

Take pain relief as directed on the packet.  If you have paracetamol you can crush that and gargle with it and then swallow.  Lemsip may be better for you as it is made up into a warm drink.  This will help to bring down the fever and lessen your throat pain.  Make sure that you drink plenty.

would it hurt to pee if I had a bladder infection...

today most of the pain has subsided a bit and my penis is only slightly irritated but is still very red at the tip.

still fevered... my head feels like its actually burning..

I feel like if I had a bad infection then I would be very sore all of the time but seems to come and go today..

also my throat is sore now... mainly on the left side.
