TESTICLE PAIN!!

Hi. I have had pain in my right testicle since 1997. I am now 24 years old. I have had numerous physical examinations and 2 ultrasound scans. I am always told everything is 'absolutely normal'. The pain feels to me like it is on the bottom of my testicle. My right testicle hangs lower; 'forwards' (bottom pointing away from body) while my left does the opposite (backwards and in towards body). Also I am positive that the epididymus on my painful testicle is at the front rather than on the back (as my left one is). Does this sound normal or could it have become twisted in some way?!? Either way I'd have expected urologist to have noticed if something was wrong. I am sure pain is not attached to testicle but sometimes struggle to locate exactly where it is stemming from. Pain sometimes radiates into my right groin (particularly when I stretch the muscle). Pain is made worse by activities such as driving, lifting and running. It always goes away when I am sleeping and comes back during the day. Sometimes I hardly notice it for a few days then it comes back! My scrotum on the affected side itches quite often too making me scratch it, bringing back the pain!
I am really at the end of my tether now! I saw a urologist last week who said 'scan is normal - nothing wrong'. I said 'there must be other possibilities about cause of pain?' He said 'not as far as I am aware'. 'I am a surgeon, if I find a lump I cut it off'!!! He was no use at all! What can I do? There are no more urologists in this town....
I do not suffer from back pain either. The pain first manifested itself after an encounter with my ex girlfriend. She didn't touch the testicles but I was aroused. I went to the toilet and felt the pain. That was six years ago. It has bugged me ever since.
Ibuprofen does nothing for me. I am from central England, U.K.
Please help me find a way to end these years of suffering. I can't get motivated to do anything because most activities cause me pain. Recently I have had a runny/stuffy nose but my urologist tells me that it is not related.
Do you know of any painkillers that my doctor could prescribe me to alleviate my pain and make life easier?
Or do you know of any possibilities of what is causing my pain? Who can I go to see? HELP!!!!

I have had pain in the rectum and left side of the testicle and penis for 4 years and up to now no doctor was able to make the connection.  Do u think that that it is one thing that is causing both pains, or are they separate. My symptoms are heavy burning starting in the anus and the whole portion of my left testicle including the penis and below the testicle.  NO swelling is showen, every test shows normal except I have severe spasm in the rectum.

Without examining you, it would be difficult to say what it is.  If you have been evaluated by a urologist, then I would assume some kind of scrotal ultrasound was done to evaluate for torsion, cysts, epididymitis or any kind of mass.  

If everything was truly normal from a urological standpoint, you may want to consider other causes that can cause your discomfort.  

The first would be some kind of prostate infection.  This can be tested by a rectal exam to evaluate for any prostate masses or enlargement.  Prostatic secretions can be expressed and cultured.  

I would also be evaluated for hernias - pain from these can radiate to the testicle.  If a hernia cannot be felt, I would suggest a pelvic CT scan to better evaluate the affected area.

Followup with your personal physician is essential.
