Erectile/Urinary Dysfunction

I am a 24/m. 6'0" and 215 pounds.

I have a mild case of narcolepsy and a mild case of sleep apnea. I'm taking about 40-60mg of Adderall per day. I'm also taking Remeron 30mg and Klonopin 1mg for depression/anxiety issues related to my sleep problems. Also taking 40mg of Nexium right now for acid reflux problems.

My last physical was in July 2007. I had blood and urine tests done, but everything came back normal. Although I didn't realize it, my erectile/urinary problems had already begun at this point.

Over the past 6 months, I've noticed that my penis is smaller - especially in a flaccid state. Sometimes I have a difficult time grabbing onto it when urinating, but I can stretch it out and that usually helps. On occasion, my penis becomes momentarily inverted into the foreskin, but this isn't everyday.

My sex drive is much, much lower than it used to be. Although not impossible, I have a difficult time getting an erection. I have not been sexually active during this time.

About once or twice a month, I've experienced a persistent urge to urinate all day long, even after I've just been to the bathroom. This lasts for a day or two, then goes away. A couple weeks ago, I could smell a foul odor coming from that area.

I've never experienced any pain, passed any blood, or anything of that nature.

I should mention that I've experienced an enormous amount of stress with my job during this period. That's why I'm taking the Remeron and Klonopin.

The only doctor I see on a regular basis is a sleep doctor who is a psychiatrist. I've been reluctant to share my urinary problems with her because she's not familiar with urology. At the same time, I've been too embarrassed to see another doctor, even though I realize this is irrational.

Any idea what might be wrong? Can I see my family practice doctor to have this treated, or do I need a urologist?

if the frequent urination preceeded taking these medication, i'd wonder about other reasons, other than the meds

i'd go see a urologist and have them examine you, eg. prostate, etc

I forgot to mention a couple things.

When this first started, I was not taking the Remeron or the Klonopin. After starting these medications (about 3-4 months ago), I noticed no difference whatsoever in my erectile/urinary problems...at least no change that was sudden enough to notice.

Come to think of it, the frequent urination has been a problem for longer than 6 months. I mentioned this at my physical. He thought it was related to the stress I was experiencing at the time.

The urinary problems I described earlier (except for the shrunken penis, and loss of sexual desire) come and go. Today I have experienced the urge to urinate all day long, but when I do, little flows out. This sort of thing lasts 1-3 days and then goes away for a time.

On the Remeron paperwork provided by my pharmacy, it lists the following under side effects:
Urogenital System
frequent: urinary tract infection

infrequent: kidney calculus, cystitis, dysuria, urinary incontinence, urinary retention, vaginitis, hematuria, breast pain, amenorrhea, dysmenorrhea, leukorrhea, impotence;

rare: polyuria, urethritis, metrorrhagia, menorrhagia, abnormal ejaculation, breast engorgement, breast enlargement, urinary urgency.

since the frequent urination & foul odor is gone for a couple weeks, i suspect there's no infection

the medication can be the reason for impotency

your family practice doc can treat this, i'm a fam pract doc and see problems like this often
