I'm another who gets this exact same problem and have done on and off for at least 4 years. It used to last a day or two and then go away for months and affect both sides. For the last month it has been there every day. It wakes me up at about 5 or 6am and it wont go away untill I get up and get on with my day. I have a lot of muscle injuries, and know this does not feel muscular, Im sure the pain is kidney related.

For the past 3 months I have been going to sleep and waking about 5 hours later with lower back pain. It seems to subside after I get up and slowly walk around, however it is taking longer to disappear and I find myself having to sleep in an upright position and take pain relief. I have a high threshold to pain but it really is causing me a lot of discomfort. I have a vaginal fistula and experience lots of discharge from my vagina. I have had an ileostomy for the past eight years due to ulcerative Clotitis and I have has many repairs of my fistula over the years without success.

Severe pain in the lower back could be due to wrong posture, nerves being compressed at the vertebral level, spondylitis of lumbar spine or in females due to vaginal discharge.
This needs to be investigated further by X-rays and MRI etc. I think you should discuss these possibilities with your doctor. It is difficult to comment beyond this without examining. Please let me know if there is any thing else and do keep me posted. Take care!
