I agree with the workup thus far. The cystoscopy would rule out anatomical causes like a urethral stricture or polyp.
I also agree with the extended course of antibiotics.
I would consider a transrectal ultrasound to image the prostate. If an abscess is present, it may need to be drained as antibiotics would not be effective.
This option should be discussed with your personal urologist.

if your doctor is running out of options, its time for a new one. This pain is a problem that ofen wont go away or "burn out" as some doctors put it, its essental you get him treated by a doctor knolageable in the subject. If you need to go to a major research hospital i recomend it. I suffered with this condition (its called Orchialgia, or testicular pain) for 15 years before the real problem was found and i was cured of it... Find a doctor experanced in the subject.
Was the Epididymitis and its cause cured? removed? the pain in the penis can be and is most likely refered from the testicles. Epididymitis can cause a scar tissue build up that wont go away. Is the problem bilateral?
Thanks, it annoys me that the doctor claims he is running out of "options" , he has a follow up next week with him, see what happens, I am not sure what cause the Epididymitis, the doctor said from lifting and the urine travels back into you somehow. Yes I think it is on both sides but am not sure will ask him, One thing is for sure I do not want him going through this for another day let alone 15 years, how did they cure you?
I was an extream case and had damage that made me sterile and not able to produce testosterone, so my cure was off the main list of solutions but relatively simple, but i dont want to scare you so im not going to go into it.
On the other hand, a epididymitis can be removed and fixed, or a denervation of the affected area can be done. Nerve blocks provided me temporary relief. You need to get a urologist who is very experienced in this subject, there are lots of options and since i dont know your details i cant say what they are.
