Pinch/Itch feeling at tip of penis

Hello I havent posted on this site for a while now. I have been having this pinch/itch feeling for two years now. I have tested for everything. HIV, HSV1&2, Chlamy, Gonno,Syphi and thric and all have come back negative. I have seen two urologist who all say nothing is wrong. I have spent over $200 on this site asking for expert opinions who all say its not an STD. I have had a cystoscopy which retuned negative for kidney stones. It doesnt bur when I urine. I have no bumps, rash, lesions or foul smell. I have taken all the antibiotics I can think of. Can anybody help me with a solution or something I can try?

You must have read my post from earlier this week.  I've had this for over a month...started with an irritation with friction from my clothes and just gotten progressively worse.  Have not been able to wear underwear for the past week...not joking and not trying to be funny.  I've seen my regular GP, another doctor at the practice and Urologist...all took urine test and I had full blood panel from my annual physical two weeks ago. Even had bacterial STD test last two weeks ago..everything looks normal and no reason for concern the doctors say.  On my third week of Cipro, but not having any impact.

Sitting here, I have on no underwear because it's too damn irritating.  I'm eating Advil like popcorn; its the only thing that helps.  The past 24 hours I have waves of pain that feel like someone pinching the opening of my urethra with their finger nails...twice I've  dipped the tip of my penis in ice water...it helps but have to do it in private and taking the day off from work.

Advil helps.  Doctor told me not to examine myself unless I have to because that makes it worse.  He advised I put Antibacterial cream on the tip, but not into the urethra.  Has no impact.  The mental anguish is almost as bad as the pain because I can't figure out what the hell this is.  I'm married and monogamous for over 20 years but found myself at local health department getting STD test, with zero reason to be there but there is something so wrong and the doctors have no clue that I'll investigate any potential causes or cures.  Wife is getting concerned because we have healthy sex life, but I've been putting her off for two weeks and she's suspect as to why.

Anyone else that has constructive feedback, please post.
