Problems with foreskin tightning due to scarring

ok, i have a problem where i had difficulty pulling the foreskin of me penis back, or even at all. this problem has gone on for a number of years, but i haven't had the money nor was health insurance offered at the places i have worked at so i haven't gotten it fixed for a number of years, but it has gotten to the point i can no longer pull if back regularly to wash it, so i need to get this fixed, despite me not having the money to do it. the skin at the tip of my foreskin when my penis is not erect is very swollen, and has two lacerations on the side that have deleveloped into scarring due to constant tearing due to when i pull my skin back to clean it, when my penis is erect the skin becomes sore from it. i have researched some and found that steroid creams have worked in similar cases, but i dont know if it will work in mine. i want to avoid circumcision due to loss of sexual functions that the foreskin protects. i would like to do have this fixed if i can, and would like for some advice.

Hi,

It is true that steroid creams have been used to treat phimosis and paraphimosis.
The recommended duration of treatment is 4-8 weeks, twice daily application of low-dose steroids such as hydrocortisone or betamethasone.

"A number of studies show that phimosis can be safely and effectively treated by the application of topical steroids in 80-90% of cases.[ 9-16 ]. Betamethasone cream 0.05% should be applied to the exterior and interior of the tip of the foreskin 2-3 times daily. The treatment should be discontinued as ineffective after 3 months if the foreskin has not become retractile during this time.
Conservative Surgery
A number of plastic corrections are available for the adult or adolescent non-retractable foreskin.[ 19-32. ]. These include preputial plasty, in which a dorsal, longitudinal incision is made through the constrictive band of the foreskin. The underlying tissue is spread with artery forceps to expose the Buck's fascia and the incision is closed transversely with absorbable sutures. This procedure has less morbidity than circumcision, and allows the prepuce to be retained."

Thank you for your help. I will go see a doctor about this when i can. I hope i can get this resolved without circumcision, and if i do have to get something like that, is there a type where i wont have to have my foreskin completely removed. i have seen a method called a dorsal slit incision which is what i think you are recommending. The method is like circumsision, but the foreskin is sutured under the bottom of the glans, making what looks like a turkey neck. I would rather have my foreskin as normal as i can, and that method is almost the same as circumsision. i am sorry to be such a bother with this, but  i guess you can tell that this is a very important matter to me
