Please help. Penis has been hurting for years

Heres my story:

4 years ago i tried too hard to urinate right after ejaculating, whilst also trying to rub out the remainder of my semen, and at that moment i keeled over and felt a massive stinging burning pain.

For the past four years this pain has been present. Ive been doing whatever i could to fix it up, nothing really gets rid of it.

The most succesful treatment has been myfoscial release therapy, which has lessened the pain tremendously, however there is still intense burning pain with urination and soreness with ejaculation, both of these feelings linger for minutes to hours to days after i do it. Also if i go too long without getting another treatment, it all comes back. So i fear it may not be fixing the issue, its just soothing my pain.

Here is every problem i could compile about myself to maybe help find an answer:
I have a pretty bad fungal infection on my foot.
I also have some jock itch on my crotch, i believe it may have recently spread to my rectum, as recently that has started to burn with bowel movements.
In the past few months my sperm has turned yellow.

I have been tested for every STD known to mankind, all negative. Ive only ever even had 2 sexual partners and they were both virgins.
Ive been tested for fungal and prostate infections, both negative.
All of these tests were done about 2 years ago.

Please help if you have any idea on what may be going on, this is the most frustrating and painful issue i have ever dealt with in my life.
