Tingling Sensation on Scrotum. HELP!

So i had a panic attack almost 3 weeks ago and I've had a whole SLEW of problems, each has lasted a few days then dispersed, but the last 4 days i've had tingling sensations on my scrotum, sometimes at the back near the area between my scrotum and my anus, and sometimes at the base, but usually spreads out in a small area, and it seems that when it happens, my scrotum contracts, and becomes tense, like it's cold, and then retracts and becomes less tense, like they're warming up, but this happens at least 10-30 times a day, sometimes it lasts a few seconds, other times it lasts longer and more frequent, sometimes 2-4 times in a few minutes.

it seems to have decreased in severity and length the past few days, but i'm still experiencing it. i haven't had sex since Nov. 09, and this is the first time i've ever had issues like this, and i know she was clean, she's the only person i've had sex with, and I was on medication at the time, so it only last a few minutes, since I couldn't keep an erection. There is no pain or other symptoms, just the tingling sensation. And when I'm sitting down with my boxer-briefs against the back of my scrotum, it feels like it's rubbing against it, it's really hard to explain, but it's only when i have the tingling sensation and my scrotum seems to retract and become tense, so that's probably why i feel the rubbing. I'm not sure what to do.

And today, I masturbated and when I took a **** shortly after, I was still semi-hard, and it seemed like I had a minor discharge, but I think it was left over sperm, because that's what it looked like, but it was yellow. And like I said, no pain or anything, I mean I had to strain a little since I had just ejaculated, but other than that, I've been fine. Please help, thank you, I'm desperate. And broke. And no insurance.

Ok so now I'm having some issues urinating, and the tingling sensation is still lasting, it seems to have gone back to how it was before, more frequent it seems like, especially when I lay down.

It feels like i'm having weird muscle spasms on the inside of my penis, like near the urethra, or the urethra itself, like at the base of my penis, I've also been having to go a little more than usual. I don't understand what's going on with my body, I have no insurance, how can I go see a Urologist?

Also I forgot to add, that I have a hemorrhoid because my anxiety has really messed up my digestive system, as well as a COMPLETE diet change on top of the anxiety, so I had been straining for awhile, but now it's not really bothering me, but I've been using a hemorrhoid cream as well as taking epsom salt bathes, could either of those cause itching?

AND as well as the scrotum tingling, there is a certain spot on my mid-lower part of my back that get's the EXACT same tingling sensation as my scrotum, sometimes at the same time, but usually separately. Could they be related? Or is this just a symptom? I noticed the tingling about the same time I started using the cream and the bathes. Thanks.
