mild Urethral burning and flat urine stream

Hi,

I am a 27 yr old male.For the past 2 months , I am having this problem of urethral burning/discomfort and my urine stream is narrow/flat at the beginning and normal then onwards. I donot have any pain during urination , no fever,  no chills, no discharge, no warts, no lesions. I have done a urine  and blood test which dint show any infection or microorganisms. From past one week i ve mild back ache too.  A simple classification of my symptoms are :

mild urethral burning/discomfort,
flat/thin urine  stream which is normal towards the end.
mild back ache since one week.

I have done the following test , and results are normal:

- blood  and urine test : normal, no microoragnisms found
-penile doppler- normal no plaques and normal blood flow
- abdominal scan:  internal organs looks normal
-Semen culture : normal, no microoragnisms found

He has also prescribed me cefoprim , which dint help much. He suspected it to be urethritis , hence asked me to use neomycin into the tip of my penis . but  no positive effect

I am very nervous about this and cant seem to find a solution to this. Can some one please advise me on this . Not being able to find the cause  is quite frustrating me :-(

Hello,
Since you are having mild back ache along with these symptoms, first of all any kidney stone along with any ureteric stone should be ruled out. Also any obstruction in the urine flow and prostatitis and urethritis need to be ruled out. I’ll suggest consulting a urologist as you might need furthest to evaluate the cause of your problem like urodynamic studies as they help in finding out if there’s an obstruction in the flow of urine Cystoscopy, IVP, Abdominal ultrasound and CT scan of the abdomen.
Please don’t delay it as you’re quite young and shouldn’t have such problems at such a young age. I sincerely hope that helps. Take care.

Hello,
Since the urine culture has come out negative then urethritis and UTI are ruled out. Also the abdominal ultrasound has ruled out any kidney stones. You do not need any MRI. Cystoscopy is the most confirmatory test for any ureteral strictures as well as any enlargement of prostate. So please get it done.

I have already met an urologist you have performed the followin tests on me  and the results were normal:

blood test and urine analysis: normal
penile doppler: normal blood flow and no fibrotic plaques found
semen culture: no microorganisms found after 48 hr culture
urine culture: no microorganisms found after 48 hr culture
abdominal ultrasound scan:  normal

My symptoms are:
mild buring/discomfort in urethra which has now spread to urinary bladder and now mild kidney discomfort/ache(since last 1 week).  The discomfort is mild and sometimes i feel its gone only to come back again after sometime.
urine flow:flat stream in the beginning 1 inch and then normal downwards (my urologists says its normal and could be because of urethra alignment. I have noticed this only  since the past 2 months.) I donot feel any loss of urine pressure or incomplete urination

I donot have any lesions, cuts, discharge, sores, no blood in urine or semen, no fever, chills , no nausea or vomitting.Infact my symptoms dont match with any dissease that i searched so far,this makes me even more worried. At this point my urologist is not suspecting  any uti or urethritis since both semen and urine culture came clean and no related symptoms either. But just to be safe he has prescribed me with cefoprim and  currently on pruflox, neither of which has helped me.  I feel like this is not an urethral infection or urethritis problem.

Sorry to write such a long mail, but you and my urologist are the only people i have discussed this issue. Sometimes i feel all this problems are just in my thoughts since all tests till date came normal and  no other serious symptoms.

I have a cystoscopy scheduled for next weekend. I am totally nervous about it. I hope it would be painless. I shall do an IVP too before that. Will ask my urologist to check my prostrate too. I just hope that this is nothing serious since the symptoms i suffer are all mild.

Just would like to know if the abdominal scan which  showed my internal oragns as normal is enough or should I go for MRI or CT scan too for further investigation?
