The major concern would be the transmission of sexually transmitted diseases.  Since there is commonly no barrier (i.e. a condom) for protection, any type of genital disease can be transmitted - gonorrhea, chlamydia, HIV, herpes, hepatitis, candidiasis (rarely), genital warts etc.
