Unsure if I have UTI or yeast infection or both or something else. Please help!

Hi. I have had problems with UTI's since I was about seven years old (33 now). Sunday January 11th I started feeling one coming on, so I took a Levaquin 500mg, as per my urologists orders (I have on hand to take when I feel one coming on and also to take before sex, but there was no sex this time).  The next day I woke up feeling so much worse than the day before.  The pain and burning from urinating, urgency, frequency, fever (102), throwing up with traces of blood and diarrhea, not to mention seeing particles floating around in my urine told me to go see my urologist.  I went to him and he took urine for a urine analysis and culture and also did a bladder sonogram which he said looked good as well.  Also I should mention I had a CT scan a few months back and all was well; no stones.  He told me to continue levaquin for five days and he also gave me pyridium.  I finished both levaquin and pyridium and still was having symptoms, so I called the urologist and was told that my urine came back fine but it could've been masked because I was already on antibiotics when I came in. So I went in again on Monday January 19th to give urine and that Friday I called for the results and again I was told all is well even though I am still in pain.  On Sunday the 25th I called his answering service because I could not take it anymore.  I was still having abdominal pain, burning whether urinating or not, urgency, frequency, fever (101), nauseous but did not throw up and very dizzy and lightheaded. At this point I have been off Antibiotics for 10 days.  The Doctor on call was not my regular doctor; she is a part of the same group at a different location.  She was able to see my chart online and disagreed with the first urologist that my white blood cell count was high the first time and significantly went down the second time.  I asked if she thought I have IC and she said no. She believed it was a UTI from past experiences with patients.  She suggested I take bactrim, which made me vomit profusely all night long, so I stopped taking it.  She then called in macrobid for me but I haven't taken it as of yet because I really cannot eat and I know you are supposed to eat on antibiotics.  She told me if I am not finding the antibiotics effective or can't keep them down that I should check into the hospital and receive them through IV.  (I neglected to mention that on the 12th after I saw the uro, I saw my PCP.  I just happened to have an appointment that day, I wasn't going because of the UTI.  I told him all my urinary issues though and he said if I don't feel better soon he suggests I go into the hospital, not only for the UTI but he was scared of dehydration from diarrhea and throwing up).  The on call urologist also suggested I see her for a cystoscopy, but not until the infection goes away.  Fast forward to right now and I am on FIRE down there.  Everything hurts; the inside of my vagina, urethra, clitoris, and my outer labia (lips) are very red and swollen and itchy.  I have the slightest bit of discharge.  It isn't the thick cottage cheese like kind when you get a yeast infection, as I have had them before but no where near as much as UTI's. I just see some slight discharge in my underwear which my gyno has always told me is normal.  So now I don't know if I have a UTI or Yeast Infection, or both, or did the antibiotics cause a yeast infection.  I want to know what you think it is and what my next step should be.  Should I go to the first urologist who insists everything is fine? Should I go to the second urologist who seemed more willing to help but probably cannot see me for an other week or two? Should I go to the gyno? Should I buy at home UTI and yeast test strips? Should I go to the ER? Also, I have not been physically examined at all.  I know it's probably necessary but I am in so much pain I am scared for anyone to touch me. Having been prone to UTI's, trust me that I know to wear cotton undies, wipe front to back, pee after intercourse, and so on. I also know not to drink alcohol or coffee or eat chocolate, tomatoes or citrus fruits. I have not had sex since October and before then I cannot even remember when I had sex last.  I am married so I do not fear any type of STD.  We just don't have sex much because of my issues, but like I said, this came on on its own. If you can please give me some insight and advice I would be so grateful.  Thanks so much for time, patience and understanding.

Sound like you should go to urgent care.. I know it's uncomfortable but you need immediate relief. For home relief sit in tub with cool water and some apple cider vinegar, if you have yeast it will help. Also, you should try to help your immune system. I recommend go on probiotics from a natural healt store ASAP. All those antibiotics are killing good bacteria and yeast infections come next. Also take some apple cider vinegar to balance your PH levels and prevent infections.  Garlic and black elderberry will help you fight infections.. Avoid sugar at all cost, carbs such as white bread should be kept at minimum. I've had similar problems in the past and making lifestyle changes does help. Preventing the infections is hard but if your immune system is good it will fight infections for you

After two trips to the ER, urologist, gynecologist and tests (renal ultrasound, pelvic sonogram, transvaginal sonogram and ct scan), the only things that were found were some small ovarian cysts which I was told is not what's causing my pain.  Right now they want me to try and sit tight, but if the pain persists I was recommended to get a cystoscopy and mri of spine.  It's just been diagnosed as chronic abdominal pain because no one knows where it's coming from.  But trust me, the pain is there!

Thanks for your response too.  The uro told me to take salt water bath but I don't have a tub so I just poured it between my legs on the toilet.  I drink apple cider vinegar.  I put a few tablespoons in a small glass of water and drink it.  Do the same with baking soda but like 1 tsp.  I also use Cran actin and vitamin d.  I ordered d mannose and UTI clear and this stuff the uro told me about.  I take acidophilus chewables because I have a prob with pills.  I think it's one billion organisms,  is that the right thing? I heard uva ursi works but destroys your liver and Indian gooseberries, however I don't think they're in season.  The last time I went to urgent care all they did was prescribe for me, no exam.  And they suggested I go to a uro which I did.  The only reason I didn't go to a uro first was bc it was a Saturday night.  What form does black elderberry come in? Garlic is not happening, as I am highly allergic.  Lately when I have been eating it has been starch like a few crackers or a piece of toast.  I know carbs are bad but I feel I needed it to soak up all the acid I my stomach and also I am a vegetarian so without carbs I basically starve.  My immune system is pretty crappy.  I get sick often and I almost always need antibiotics.  I never have anything viral.  I think all of the antibiotics I have had through the years have either made me grow immune or make me sick.  Thanks so much for your feedback, I really appreciate your kindness.

Thanks for your quick response.  I believe the throwing up and diarrhea were side effects for the antibiotics and the blood in the vomit only happened once on the 12th.  I think that will remain a mystery. Any vomiting since has not contained blood.  I don't think I should take antibiotics before I see a dr because if I have a yeast infection, the levaquin or macrobid will make it worse. I wonder if I never had a UTI to begin with and it was always just a yeast infection.  The discharge is white but like clearish white not opaque and not thick and not a bad smell.  Also antibiotics require eating, which I am not capable of that right now.  I have reglan for the nausea. I asked for zofran but was told that it's really for cancer paitients.  What does a kidney panel run entail? I take cranactin multiple times a day and I drink a gallon of water a day. Thanks again.

It could be either of those things but that would not explain the blood in the throw up or diarrhea. That is what concerns me!  The antibiotics will help fight whatever infection you do have and you should take them. Go to your PCP and have them prescribe zofran it is an antiemetic that will help with the nausea and vomiting, and it is safe to take with antibiotics. The ER is an option but you will be checked in and they will star you on antibiotics IV and will keep you there at least 3 days. This could be a parasite as well, if the discharge is a strange color (yellow or green or gray) it could be indicative of this. I think the discharge needs to be cultured as well as another urinalysis before they put you on anything else. Finish the antibiotic and have a kidney panel run as well this might tell you if it is more serious then just a UTI. That's the plan of care I would prescribe as well as drink a ton of water and cranberry juice.
