suffering bad mid cycle pain for years

I have suffered bad mid cycle pain for years but in the past year it has been getting much worse.  I have had mittelsmerz rulled out so its not that.  I have the pain for about a week and it gets worse and worse each day till it vanishes.  It does ease it just vanishes.  The pain in all through my pelvis with a lot of swelling in my below region.  I bloat out to look about 4 months pregnant.  Doctors havent done much to look into it and no one can say what they think it could be.  

The pain this month was the worse so my Gyno sent me for an Ultra sound which was done the day after all my pain stopped.  The ultra sound showed several follicles in the right overy measuring up to 22mm each.  No corpus luteal cyst seen on either side and 50 cc free fluid in the pouch of douglas.  

Is there any ideas on what could be wrong as my gyno has said my only option left is to go through a fake menopause to see if it helps and if it does then they will remove my overies which I dont think I am ready for until I know whats wrong

I was given a laparoscopy years ago as I suffered secondary infertility but they didnt find anything then.  I also fell pregnant about a year after this with ID twins so they thought the problem would go away but it has gotten worse the past year.  Birth control does stop the pain but I seem to have side effects to every form of birth control there is.  My husband has had a vasectomy so there is no chance on falling pregnant so that is not an issue.  I only started suffering from this after I had my second son and started trying for another which was about 6 years ago.  My hormone levels have never been checked either and I noticed this time when the pain was the worst its ever been my breasts were also sore which hasnt happened since I was pregnant with my twins.

The cysts alone can cause pain OR WORSE Tortion(rotation and oxygen deprivation,) if you think at all that is it an ultrasound can diagnose oxygen dep though talk to your gyn if you worry and it wasn't checked. Have you been checked for endometriosis?
