Low testosterone and sperm count

I was tested over a year ago for total T level and it was at 238. I was prescribed T injections every two weeks by my PCP. I did a semen analysis as me and my wife were thinking about having kids. I had a zero count. My last SA was done a few weeks ago and now there are a few sperm but nowhere near fertile levels. I have also done several LH, FSH, prolactin, and Free T level tests. My free T levels were always in the normal range according to Kaiser but the total T level has always been low. What should my next step be? My last total T level was checked yesterday and was at 104 and done in the morning. I think I need to see an endo but need advice...
