Anal Leakage

(Over 2 years) My symptoms include:1)Anal leakage: More after a bowel movement. If I wipe it clean, I can go back an hour later wipe again and find residue on the paper. This can go on for quite sometime. 2)Incomplete bowel movement: I usually feel like there is still some left but it won’t come out. 3)Irregular Bowel Movements: Some days I can go up to 4 times others 1 time. The consistency of the fecal matter is always different from full, watery to stringy. 4)Anal Itching: I think this is due to the leakage, but there is what feels like a small bump just out side the anus. Guessing its Piles. 5)Since I wipe excessively, I due on occasion get bright red blood on the toilet paper. I have also noticed that if I have to go but hold it in; When I do go, higher chance of getting blood on the paper. 6)Spicy Foods and Dairy, if eaten in large amounts can escalate my conditions causing bloating, and diarrhea that often turns to clear water. 7)I have been passing gas more frequently. 8)I have been treating the area in between my scrotum and inner leg for jock itch that never fully goes away. It started when I sprayed some old cologne there and I have been getting a fowl smell in that area. Jock itch creams are a temporary solution but the smell comes back if not treated daily. (Treating for 1 yr now)
Profile: I am 28 yr old male that smokes. I drink a lot of coke and juice. My eating and sleeping habits are not routine. Little exercise, 190 lbs. Your expertise is greatly appreciated. Any ideas on what my problem is, and what I can do to treat it?

Infection, irritable bowel disease, anatomical abnormalities, or anorectal dysfunction can all lead to the symptoms you describe.

Obtaining stool cultures to rule out infection and a colonoscopy would be a reasonable, comprehensive approach to evaluate for many of the major causes.

More specialized testing, like analysis of the stool for malabsorption, as well as tests to rule out anorectal dysfunction (i.e. endorectal ultrasound), can be considered if the symptoms continue.  

If the tests continue to be negative, irritable bowel disease can be considered.  An approach to this would be increasing the amount of fiber in the diet, as well as antispasmodic medications.

These options can be discussed with your personal physician.

Old cologne just irritates your skin.
Try using anti bacterial soap and gently wash the area several times and dry it well,if it were bacterial infection.
If it were fungal infection,you need to take that acid phil culture either in tablet or in yogurt,also try applying some of that yogurt on skin for several days and see if it helps.It should have a cool smoothing effect.
You also need to keep that area dry and ventilated to avoid recurrance,which means no tight underwear where air does not circulate.
