hi. i think that burning pain in your chest and down your arms are symptoms of spondylosis. i am a radiographer and i have often seen patients with these symptoms are often misdiagnosed with cardiac failure and other pathology. A simple xray of the cerical spine with oblique views included would exclude any signs of nerve compression and spondylosis.
especially taking into consideration that symptoms persist predominantly in the mornings this is probably because of the uncontrolled positions and postural variations in deep sleep which could result in muscular spasms related to established spondylotic changes.

It sounds the the radiologist saw a prominent left hilum which a place where its bronchioles and vessels converge. It's just a descriptive phrase, so it doesn't give a diagnosis. Hilar prominence could be nothing, or it could be inflammation, or it could be a sign associated with sarcoidosis, or it could be an infection, or it could be cancerous. Considering your age reason your Doctor is worried as he wants to rule out any such causes.
Probably considering your age CECT chest may be indicated to be in the safer side. Howver, your physician will be the best judge.
