Justin that is not that high of dosage, but were it high, after three years you would have become tolerate to that dose. Any dosage will become ineffective over during a time. When I got hurt in 1990 and was almost bedridden during, the pain was out of control, i got bad advice by first neurologist( He told me to go to bed for a month, a very stupid thing to do. I needed Physical Therapy right away, I know now or at least about a week or so later.) I didn't get better because I had torn muscles in my back and I layed around and that made me weak. It caused my spine to get worse I think and then I got bulging disk. I may have had them already. Had he ordered a cat scan and I mean from the wreck but during that time, I might of gotten the PT and done well. that month I am sure made me worse because in1994, I had an operation after the mucles had healed some , and I was made to get out of bed a couple of days after I was able or rather lucid enough. .This was the sixth day after surgery. If I could walk the floor I could take a shower. If I did it twice I could go home.. or when. I think these sentences are running but is 648 am and I have been up , hardly sleeping as it were and not the best of times. Anway,my time of incarceration as I like to call it was 14 days. 5-6th day doc came and asked if I had lifted my body with the triangle above me to which I said no. I had just started eating the day before. He took his hand , no one had told me to lift me with that device and pushed my back upward like somone poking his stomach out. I thought I would die, so I walked to shower and I looked at my hubby and said, " I am getting out of here before they kill me " lol.....they also put a piggy back line on my IV line and the meds for pain was going to there and laying. Nurse was probably taking it lol. I was getting saline and she was getting morphine.. Joking but happens.  Anway again, next day she tells me it is going in the floor, and if I were able I would of bitten her ( laughing)  now but I was in agony. I was cut on back under ribs to my anus.. DR said was as serious ad open heart sugery........OkaySo he did that I left.I had to walk 2 miles in about 30 minutes by week 6. I did and not only that but I did the exercises they gave me twice a day. From this I know laying about is not good. I did really well. He said he would give me 80% and I got a 100. He said I did it. That if his patients would maintain their weight especially their stomachs flat or near, they would be. This doctor taught Neurosurgery at Chapel Hill University. He was head of it. His partner was vice. He is a genius.. but huiman He was great  as well as his bedside manner and he was empathetic to a degree. I digress. Anyway, now that I know all that I went through , and I got hurt 12  years later in my later 40's and it ruined it and made many other problems, so the difference is pain management. I was at tolerance long before the operation. Meaning the meds no longer worked like they should. So I don't know how I stood it. Well I did not. I had to go to ER,and I took more. To people I think they thought I ws just seeking drugs. They didn't take into consideration how long I was on the meds 4 years. I had been to 4 neuros and been to therapy at three or four clinics associated with their doc I think I went to chiropractor for 4 years. The first 3 months every day and sometime he would put me in a room and leave me for hours . It was hellish.
Now they know more about pain or seem to and I can't find surgeon yet that I trust to cut me. My doc moved to another state. I have metal . pins and screws in my L3-5 with fusion. There is my problem. I have to find a doc that can redo this, scoliosis and I have spinal stenosis , three disk that are bulging, many limbs  or parts going numb and pain going down buttocks to calves or wherever. Honestly I have not idea what will hurt where or how but constant and persistant like long ago. Now I get help with meds. However if I ever find this doctor I trust I will get surgery and do all I need to to get well. I will become the poster elder for his group./ I am 53 and all say I look 40. I dont know what that means. They Lie well probably :) but You need to go to a Pain Clinic. They understand pain and they will set you up with any other type or care you need. People seem to be afraid they will not give them but they go by different specifics: tests; cat scan, MRI, Xray , then they can see you, and they will talk with you. They want to help you. If you take your meds as needed they will help you.But if you can be fixed.. See Dr Su in Durham NC. I a m sure he is as good as my doc.. Maybe better..I would never take a pill over being independant again. I want my life back. It will be more difficult this time , but i have exercised since I was 18 nearly daily.. god bless you

Based on the information provided, your current use of opiate pain medications is quite high and it would appear that your body has developed a tolerance to this approach of handling your pain.
You don't indicate what problems resulted from your incident 3yrs ago (did the confrontation with others occur at that time or later?)
Pain management is usually recommended when other treatment has failed to produce results and the danger of prescribing higher doses of current medications or even stronger opiate/narcotic medication is not deemed advisable.
There is a pain management forum on MedHelp that might assist you in greater detail than we are able to do on this forum.  Just go to the list of forums and scroll down to the Pain Management forum.  Most of the individuals participating have been coping with chronic pain issues and would be able to support you in your decisions regarding further medical care.
Best wishes ----
