Itchy Nipples

I'm a 22 year old female and have been having itchy nipples for about 2 months but they became worse recently. It's flaky and scaly, and there are regular yellow secretions that dry up quickly on my bra. There are also red bumps around my nipples and they feel hard and hurt when I press them. I have them on both nipples though the left nipple itches more.
Is my condition severe? I'm unable to visit a doctor until one month later. Would it be better to visit a dermatologist than a general practitioner?

I guess I should clarify my comments.  Regarding your question of whom you should see, I would first see an OB/GYN rather than a GP because they are more familiar with the issues with breast changes.  I wouldn't recommend going to a dermatologist since the problem my not be dermatological at all.  If you don't have an OB/GYN, then a GP or an Internist should be able to help.  My first choice would be to see the OB/GYN doc.

Start with either a GP or an OB/GYN.  Let them guide you to your next steps.
