MEDICAL PROFESSIONAL
I am sorry but there seems to be too many things going on here and it would be impossible to give you an opinion without seeing you and knowing a more complete medical history. You need to see an MD and  oral surgeon for a definitive diagnosis.

You smoke? smoking deprives the largely vascular mucosal tissue of the mouth and can cause tissue change.  Oral thrush (yeast infection) casuses a sore red mouth and white coating.  This can be easily treated with anti fungal meds (nilstat etc).  The polyp sounds like a separate issue.  Poor oral care sounds like the most likely cause of the white coating.  The rash could be many different things.  Oral HPV (human papilloma virus) or wart virus could be a possibility.
I would suggest you go and see a dentist and a General practitioner.  Here is a link for oral HPV pictures:

Don't stress until you go and have all the tests.  As for your kidneys and stomach I think this is again a separate issue.
