Depression especially, can be a life long issue, it can be a chemical imbalance, it's absolutely nothing you did wrong. The most important thing is that you get stable. Usually, but not always, the anxiety drops when the depression is lifted. I do recommend you get counselling and get hooked up with a psychiatrist if you haven't already done so. Finding the right course of meds takes a bit, but you can feel better.
I was diagnosed with Major depression, and have pretty much accepted that I will be on meds the rest of my life, I look at it like someone who has epilepsy, they have chemical imbalances as well, just a different part of the brain. It's really important to get talk therapy for the PTSD, drugs alone don't work that well with it, cognitive behaviour therapy hand in hand with meds can really alleviate the symptoms and retrain your brain.

Sorry this was long winded, for depression and anxiety - there is no cure, but you need to learn coping mechanisms for the anxiety. It's helped me a ton! Paraxitine aka Paxil does help many folks, but it's important to keep track of your symptoms, like the Depression tracker that you can use here. It's going to take 4+ weeks to feel the effects of the med.

This is a good workbook that has everything under the sun, you might want to print this out.

This next one is like an encylopaedia of mental conditions and information, it's good to bookmark this one.
