Determine cause of Post Inflammatory Hyperpigmentation

Hi. Can someone tell me if it's possible to determine the initial cause of PIH (if unknown), and when the event occurred that caused the PIH, even if it occurred 2-3yrs prior? Would this require a biopsy to get the histology?  In other words, if I had a spot on my arm that was PIH, would it be possible to determine if it was caused by a burn, infection, chemicals, trauma, etc. by taking a biopsy?
