Lyme, nummular eczema, ringworm, Lupus, or something else?

This rash started exactly two weeks ago,  It started as a very itchy area of skin with small dots (thought it was poison ivy).  I also had some other spots on my neck and arms that looked like poison ivy.  I went to the doctor's right away and since it was only on the left side, she said it could be shingles.  She sent a sample to the lab it was negative for shingles.  Within that two week time period the are on my left shoulder has transformed into a large circular (kind of like a tear drop shaped) area.  My doctor gave me some anifungal cream thinking it was ringworm but the cream is not helping it at all.  I was also put on Diflucan unrelated to the rash so that alone should have (?) taken care of it.  The border of the rash is raised and when inflamed (border) it itches like crazy.  The center is very smooth but some days a little scaly (perhaps from the steroid creams).  Does this look like it could be from Lyme?  I was camping for two weeks with my family out west (California, Colarado, Utah, Midwest) so I could have had a tick bite and not know it?!  I will definitely be following up with my Family Doctor or am considering calling a Dermatologist.

EM rash may not display a classic bull’s-eye (ring-within-a-ring) appearance, a fact that may be underappreciated (Atypical Lyme rash)

Since you have visited Tick bite prone areas it is prudent to bear in mind this fact.

Considering these symptoms any tick bite should be ruled out. It causes Lyme’s disease which is a long-lasting autoimmune disease in which the immune system attacks normal body tissues as though they were foreign substances. It may cause joint pain. Diagnosed by ANA titres.
