end of my tether

am diabetic but under control.  have been hospitalized twice in the last few months with a DKA which is unusual in controlled type 2 but doctors think I had a infection  prior to attack which also caused heart failure and kidney failure but I came out of hospital after  3weeks on the mend. while in hospital I had frequent attacks of non infectious diarrhea . doctors ordered a ct colongraph as a urgent measure but their request got lost so I have had to wait for a new date. .first week home I was not to bad but due to nerve damage from diabetes can only walk a few steps with a zimmer frame . I have steady got weaker and sleep about 18hrs a day have no appetite so am losing weight have either no bowel movement for days or diarrhea .my urine is cloudy and dark and I struggle to go. I feel so low . I have got soon appointments with renal-urology-cardio- cardio-on different dates but no one seems to get all my details under one consultant and I feel that I am being overlooked . any advise please
