Fraud

Is it illegal or considered fraud if a doctor forgets to completly fill out a CMN (prescription) and a DME company fills the missing information out themselves in order to service (ship) a patients products.

technically, that's not the right way of doing things.  If you are not happy with the doctor and the services the doctor provided, you could complete a complaint form for the doctor to hopefully improve services for the future and for the rest of the public.
