See an orthopaedist for an evaluation and go from there.

I'm curious to know why you feel your back pain is related to diabetes? This like having a toothache and seeing a foot doctor. All kidding aside, you have Sciatic nerve issues usually caused by a bulging disc pushing against a sciatic nerve. A good Chiropractor within 2-3 visits can realign your lumbars and alleviate the pain non-evasively, or you can opt out for spinal cord surgery, a disectomy of the disc.
