why did my little dog die

My Tommy was 14 years old, he was always coughing a lot due to a windpipe problem, this was going on for about 3 years but he was a happy dog, enjoyed his food and walks. On Monday there seemed nothing wrong with him, he had his walk, his dinner, his supper and treats, his stool was normal. At about 10 pm he became a bit uncomfortable and he was staring at me. I picked him up and sat with him for a bit. He laid down and slept. Next day I took him to the vet at 1pm. He had his walk but did not eat his b/fast. The vet said he had a heart murmur and gave him an injection and tablets. I went back at 5.30 pm because he was not getting any better, he gave him 2 more injections and told me to come back next day. I was so worried I asked the vet if he was dying, he assured me there were no signs at all that he was going to die. A few hours later Tommy died. What keeps going on and on in my head is that over the last 2 weeks he seemed to have a bit of a different cough, like something stuck in his throat, I used to give him a bit of honey and that seemed to help, I did not think much of it because he had been coughing for years and his heart was checked in May and it was alright. Should I have gone 2 weeks earlier, he could have been on the heart medication and maybe still be alive. It all went so quick, I cant stop crying, this little dog went with me every day, I miss him so much and I feel I let him down. My friends say it was old age but I am not sure.
