If you had any bleeding disorders it would have been detected in your childhood itself, I do not think so it will be left undiagnosed till this age. Secondly, as you have mentioned your epistaxis is less when you are using saline nasal spray, that means the mucosa of the nose which was getting dry before is now hydrated and so less bleeding. Please get a nasal endoscopy done to look for any haemangiomas or septal spurs. Also, continue saline nasal spray and drink plenty of water.Any more doubts you are welcome.

I havent used that nosespray in over 6 months.
It may be that nose spray!! I had to take my father to ER twice due to severe nose bleeds and he was advised to stop his nose spray and only use Saline. I can't use the nose sprays either or my nose bleeds. I will also have a problem if the weather and house are dry. I use the Saline spray and have noticed an improvement. I do have a deviated septum and was told that can cause nose bleed problems.
I will be making an appointment with my family physician to get this checked out. I use nasal steroid spray Flonase besides the nasal saline. No BP problems...they actually stay pretty low.
My septum is not deviated. I will ask that my bloodwork be done and clotting factors checked.
I would reconsult ENT and get a CBC with clotting factor labs done and your nasal passages assessed.
Any other medical problems? BP ok? You mentioned something about headaches, and I am questioning if these headaches and epistaxis are related to elevated blood pressure.
I am not sure if epistaxis cream will help or if you need cauterization for the epistaxis.
I am not sure if this is something serious or not, but it needs to be further investigated.
For one, I'd get a second opinion from anther ENT doc. Do you use nasal sprays other then saline? Do you have a deviated septum? Have you had any blood work done and your clotting factor checked? I think I would check these things out. Good luck.
